    I am 31 yrs and have pcos..was unable get pregant on my own,we started with clomid and letrozyle and did not ovulate.so my my doctor suggeted we do ivf we did the egg retreival and retrieved 18 eggs out of which 12 made it after the pgs testing we had 9 normal embyos. We did our first fet in november of 2015 and transferred 1 embryo
    and implanted but hcg was low 25 and misscarried. The second fet was in june we transfered 2 embryosand same thing happend impalnted with low hcg 19 and misscarried again.
    we did all the blood work and was told was that i was borderline with APS. The next fet was in march we transfered 2 embryos now my dr added lovenox and vaginal vaigra and i did implant but with low hcg and misscarried. What would be causing the misscarriages i would reply apprieacate ur opinion .

  2. Hi Dr. Sher. What is more important? Embryo grading or that the blastocyst is PGS normal? I have a day 5 blastocyst 3bb grade and a day 6 blastocyst 4ba grade. Can these embryos turn into a healthy pregnancy and baby? Are success rates high for these grades? Thanks for your time!

    I am 46 yrs and just had a failed IVF cycle using donor eggs. I had initially had a failed IVF cycle using my own eggs late last year.
    15 eggs were retrieved from the donor, 11 fertilized and had only 2 blastocycysts which were transfered. AMy uterine lining was 7.3 cm on the 13th day after i had taken the first dose of Lupron.I am meant to be having a review session with the doctor next week and I would appreciate any advice to guide my questions. Normal routine checks were done , thyroid etc and my levels were all within normal range.
    I am of the opinion that the results from the donor eggs were not differently from when i used my eggs. (Mine was 10 eggs, 5 fertilized and 2 blastocysts ( transferred). Sperm from my partner was used in both cases, and he has previously fathered 2 children.
    I intend to commence another donor cycle soon but would like to know what to do to improve my chances as the entire process is quite challenging for me , emotionally and financially.

      In my opinion a lining of <8mm is not conducive to a good IVF outcome. It needs to be at least 8mm by the day progesterone injections start and ideally >9mm. And yes, in some cases Viagra therapy will help.

  5. I am a 45 yr old with AMH of 1.82 and normal FSH and 10 follicles found onthe ultrasound.
    In my very first IVF cycle, my RE tried a min-stim: 2 day birthcontrol, 5-day Letrozole, 5-day Follistim, 2-day Ganirelix and Pregnyl 10,000 unit for hCG trigger. 4 follicles found with 3 eggs retrieved and 2 embryos (8-cell and 9-cell) successfully fertilized with ICSI and transferred on Day 3. My partner has 71% amorphous semen in the analysis with some reduced 29% motility, 6% concentration, 3% linear, 7.1 ph, 1% normal ovals. I was also on DHEA & CoQ10 supplements along with pre-natal, B, 12, C, D, E vitamins & follic.

    However, the implantation failed. I am now at the stage where I have to decide if I should use my own eggs in the next cycle or use donor eggs. I have read your work about LH and its adverse effects on egg quality & development. Will reducing the LH stimulants & DHEA increase my chances with my own eggs?
    Thanks for any advice.

    • p/s: I was also on Endometrin 2x per day after the Day3 transfer and no cysts or issues with uterine lining.

      I am impressed with your ovarian reserve, however, given your age, fewer than 5% of the eggs you produce are in my opinion, likely to be genetically/chromosomally “competent” and able to propagate a viable baby.

      I urge you to move to egg donation!
