Dr Sher, I would like to know, when you thaw frozen eggs & fertilise them. If you get blastocyst embryos, can you then re-freeze them again & thaw them again later to transfer? Seems like so much freezing & thawing! Are they affected by all this? The freezing method used is vitrification snap freeze, I should mention.

I had a chemical pregnancy before,I did ivf and my hcg 8dp5dt was 27 then I tested it again 10dp5dt was 56 is it a normal range corresponding to the number of days after embryo transferred??

Hi .. Am 41 years old. Had 3 failed iui and have tried to do 2 cycles of ivf but failed at the injection stage . I have gone from having 1 cyst on my right ovary to have 3 cysts on my right ovary. With the iui I had 3 to 4 focilles but there was nothing there since I changed over to the ivf. The last iui was in March and I had one failed cycle of ivf in June and was advised to take 4 dhea tablets a day. Started again there 10 days ago and no responce to injection. Where should I go to now

Clearly this is an aberrant response. In my opinion, the most important consideration is to review and revise the protocol used for ovarian stimulation. As for the DHEA, in my opinion this is not a good idea and I would advise against using it.
The older a woman becomes, the more likely it is that her eggs will be chromosomally/genetically “incompetent” (not have the potential upon being fertilized and transferred, to result in a viable pregnancy). That is why, the likelihood of failure to conceive, miscarrying and of giving birth to a chromosomally defective child (e.g. with Down Syndrome) increases with the woman’s advancing age. In addition, as women age beyond 35Y there is commonly a progressive diminution in the number of eggs left in the ovaries, i.e. diminished ovarian reserve (DOR). So it is that older women as well as those who (regardless of age) have DOR have a reduced potential for IVF success. Much of this is due to the fact that such women tend to have increased production of LH biological activity which can result in excessive LH-induced ovarian male hormone (predominantly testosterone) production which in turn can have a deleterious effect on egg/embryo “competency”.
While it is presently not possible by any means, to reverse the age-related effect on the woman’s “biological clock, certain ovarian stimulation regimes, by promoting excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), can make matters worse. Similarly, the amount/dosage of certain fertility drugs that contain LH/hCG (e.g. Menopur) can have a negative effect on the development of the eggs of older women and those who have DOR and should be limited.
I try to avoid using such protocols/regimes (especially) in older women and those with DOR, favoring instead the use of the agonist/antagonist conversion protocol (A/ACP), a modified, long pituitary down-regulation regime, augmented by adding supplementary human growth hormone (HGH). I further recommend that such women be offered access to embryo banking of PGS (next generation gene sequencing/NGS)-selected normal blastocysts, the subsequent selective transfer of which by allowing them to to capitalize on whatever residual ovarian reserve and egg quality might still exist and thereby “make hay while the sun still shines” could significantly enhance the opportunity to achieve a viable pregnancy

hi Dr. Sher,
We had an IVF twins and one has died at w8. Second continues to live and we are at w23 now. On w20 we made a detailed ultrasound and dr saw the 1st sac died still there but for other all physical aspects all are ok. For genetic tests, we did the prena test and trisomy 21 positive detected on w21. After receiving result, immediately on w22 we did the amniocentesis and FISH result was also 98,3% 3 signal received on 21th chrosome for trisomy 21. Now we are waiting the karyotype results. Did you have any experience that amniocentesis gave false positive result due to vanishing twin syndrome and is it trustable. Is it possible sacs are effecting each other? Thank you for your kind support and sharing your experiences.
Sadly, the amniocentesis result would be reliable and in my opinion, unaffected by the vanishing twin.

I’m 42yo, had a successful IVF with life birth last year. We had 2 failures prior to that. We recently had a failed FET. I want to know if it’s advisable to try a fresh embryo IVF at my age and what is the success rate? Thank you
