Dear Dr Sher, Thanks for having such welcoming space! We did our first IVF a few days ago. I was on straight Gonal F 187.5. During this cycle my oestrogen was always low-ish, but always increasing, I triggered at 967 pg/ml with ovidrel. I ended up with 18 eggs, but 12 were immature, 6 of the 4 fertilised and they put a good 1x 8cell back on day 3. Waiting to see what happens with the other embryos. My Doc thinks I just need a new protocol, an agonist one. I see a lot of women with immature eggs, but their oestrogen correlates to number of follicles and eggs – mine didn’t, despite all the follicles looking ‘mature.’ I had this problem with IUIs and menopur 75iu injectables also. Mature looking follicles below average oestrogen. FYI – we are unexplained, 33 yrs old, ttc 3yrs, I have slightly low progesterone 8ng/ml each month, very light menstruation, amh 3.6ng/ml, fsh/lh 4.5/2.5. Can you offer any thoughts/light on this, should I see the lowish-oestrogen as a problem? Will I always have immature eggs at IVF? Is this a sign I have bad eggs? I have read your some of your articles about immature eggs, but I don’t always see the relationship discussed between low-ish oestrogen levels and immature egg at egg collection when follicles look a decent size! I would not be surprised if your high percenytage of immature eggs is linked to the protocol used for ovarian stimulation in general and the use of 25omcg of Ovidrel for the trigger…in specific. The ideal hCG dosage is 10,000U of Pregnyl/Novarel/Profasi or 500mcg of Ovidrel. Less than that, in my opinion is not adequate to mature eggs optimally.

Hello Dr Sher,
I was hoping you could give me some advice? My husband (34) and I (35) have been trying for a baby for 16 months with no joy. My tests have all come back ‘good’ and my husband’s ‘fine’. However, he has a MAR result of 95%. We have been advised that this isn’t an issue and that we should still be able to conceive naturally. However, because of our age, we have been advised to try IUI for 6 cycles and then moving to IVF if it doesn’t work. The antisperm antibodies result worries me and I’m unsure if there’s any point trying IUI. Would you advise trying IUI or going straight to ICSI? Also, do you consider 95% to be a concerning number? Thank you for your time.
In my opinion, IUI will fail. You need IVF with ICSI and all other factors being equal, that will work.
The presence of sperm antibodies reduces male fertility significantly, but does not usually prevent conception altogether. Rather, the effects are graduated; i.e., the larger the immunologic response (concentration of antibodies), the less likely it is that a pregnancy will occur and when the blood level rises above 40%, natural conception is highly unlikely to occur.
Like any other kind of antibody manufactured by the body, sperm antibodies are formed in response to antigens. These antigens are proteins, which appear on the outer sperm membranes as the young sperm cells, develop within the male testes. In the man’s own body, his sperm are regarded as foreign invading proteins and as such would normally be targeted for attack However, under normal conditions, direct contact between the man’s blood and sperm is prevented by a cellular structure in the testes called the blood/testis barrier. This barrier is formed by so-called, Sertoli cells, which abut very closely against each other, forming tight junctions that separate the developing sperm cells from the blood and prevent immunologic stimulation. However, the blood/ testis barrier can be broken by physical or chemical injury or by infection. When this barrier is breached, sperm antigens escape from their immunologically protected environment and come in direct contact with blood elements that launch an immunologic attack.
Once sperm and blood come in contact, whether in the male or female, specific antibodies are produced against them by specialized blood cells call T- and B-lymphocytes. The three main types of sperm antibodies produced are Immunoglobulin G (IgG), Immunoglobulin A (IgA) and Immunoglobulin M (IgM). These antibodies bind to the proteins (antigens) on the sperm head, midpiece or tail. The antibodies formed may be of the circulatory type (in the blood serum) or secretory type (in the tissue). This is important because high levels of antibodies in the blood serum do not invariably mean that the antibodies will find their way to the semen where they can affect the sperm. For example, the concentration of IgG is much lower in secretions of the reproductive tract that it is in the blood. Conversely, the local level of IgA is higher in the reproductive secretions than in the blood. This is an important point, which we will return to later.
Once sperm antibodies have formed, they can affect sperm in several different ways. Some antibodies will cause sperm to stick together or agglutinate. Agglutinated sperm clump together in dense masses and thus are unable to migrate through the cervix into the uterus. Other antibodies mark the sperm for attack by Natural killer (NK) cells of the body’s immune system (ie; opsonizing antibodies). Some antibodies cause reactions between the sperm membrane and the cervical mucus preventing the sperm from swimming through the cervix (ie; immobilizing antibodies). Antibodies can also block the sperm’s ability to bind to the zona pellucida of the egg, a prerequisite for fertilization (ie; blocking antibodies). Finally, there is recent evidence that the fertilized egg shares some of the same antigens that are found on the sperm. It is possible that sperm antibodies present in the mother can react with the early embryo, resulting in its destruction by phagocytic (ie; phagocytic antibodies) cells.
There are a number of diagnostic tests available to detect the presence of sperm antibodies. These are performed by flow cytometry and the ELISA (enzyme-linked immunoabsorbent assay), the Franklin-Dukes sperm agglutination assay or the Immunobead Binding Test (IBT), to name a few. SIRM, the indirect Immunobead Binding Test (IBT) is used to detect antibodies present in the blood serum, in cervical mucus or on the sperm surface.
In the male, IgA and IgG are found in the semen although there is controversy as to whether they originate locally (secreted by testicular cells) or cross over from the circulation. Antibodies of the IgM class are not found in semen.
Like the source of some antibodies, the question of the critical levels of sperm antibodies is also hotly debated among clinicians. There seems to be general agreement that blood levels above 30% by the IBT are associated with significant fertility problems.
Studiers have shown that pregnancy is highly unlikely following natural intercourse or intrauterine insemination when either the woman or the man harbors significant antisperm antibodies.
While , there have been isolated reports that administration of corticosteroids (eg; prednisone) will temporarily suppress antibody production pregnancy rates are poor. Besides, corticosteroid therapy carries with it the risk of significant side, some of which (although infrequent) can be serious. As an example, in the man spontaneous fractures (especially of the neck of the femur) have been reported in 2 % of cases. I do not recommend this treatment.
In Vitro Fertilization (IVF) with intracytoplasmic Sperm injection (ICSI) is the best option . Here each egg is injected with a single sperm and whether there are antibodies attached to the outer surface of the sperm becomes irrelevant.. In fact, pregnancy and birth rates are the same as in cases where IVF is performed for reasons other than male factor infertility. IVF/ICSI success rates are also .not unaffected by the concentration of antisperm antibodies.
*Intrauterine insemination (IUI), (sometimes referred to as artificial insemination) of processed sperm is contraindicated in cases if moderate or severe male immunologic infertility because it does NOT improve pregnancy rates over NO TREATMENT AT ALL.

Dear Dr. Sher: We just completed our first IVF cycle with 0 fertilized eggs. I was on bc for about a month and a half, then started on Menopure (150) and Follistim (150). Ganirelix was added towards the end. I triggered on day 12 with Lupron/HCG for a day 13 retrieval. I got 26 eggs, 23 of which were mature. None fertilized so ICSI was used 2 hours later. None fertilized. I was told there was an egg activation problem. Does that mean my subsequent IVF cycle will be the same or do we still have a chance for possible normal fertilization? I really want to try IVF again before moving on to DE. Age: 39. Thank you.
No! But it means you are a high responder and as such you require a very individualized approach to ovarian stimulation to try and avoid this in the future.

I am relatively new and asking for your opinion. I had laparoscopy laser ablation for endometriosis in sep 2014, had stage 4 with adhesions around ovaries that were partly removed and appendix which was removed. My Amh was 0.46. My tubes were patent but had some edema. 2015 I had IUI with Clomid 100 mg, all three times I had 2-3 follicles greater than 20 mm and last one I had 4 but all failed. I then went for ivf I was found to have a polyp which was removed AMH 0.36, FSH 12, estradiol 115 on day 3, afc 2-3, inhibin B 112, I had one IVf cycle with BCP/follistem 225 and menopur 225, and gainirelix 250, I had 4 eggs retrieved , 3 mature and fertilized, first arrested on day 2, others on day 5 and the last was morula on day 5 so they waited for day 6 and that arrested too. So my RE said that he can try again but chances are slim so I should try 3 -4 IUI again and if unsuccessful go for DE. I don’t have any coverage with my insurance so all this is out of pocket including medications. I turned 37 this month. Is there any hope with my own eggs, or IUI or should I just go for Donor eggs. I also have fibroids but had them romped, remaining are intramural none are sub mucosal. Please advice

Yes, contingent on your age, there could still be a cahance in spite of your DOR and endometriosis. Remember, the latter also mandates that you be checked for an immunologic implantation dysfunction becausae 1/3 of women with endometriosis (regardless of severity) have NK cell activation (see below).
You need a modified, robust, long pituitary down-regulation protocol. I would use an agonist/antagonist conversion protocol with human growth hormone (HGH) augmentation and would recommend Staggered IVF with embryo banking of PGS (next generation gene sequencing)-normal blastocysts, to make hay while the sun still shines.

Dear Dr. Sher,
How can a dominant follicle be slowed down? What size is too large for a dominant follicle at trigger?
It cannot be slowed down. If it stands alone and there are may other small ones coming up. I would aspirate it under local anesthesia before hCG trigger. >23mm is cystic and wont usually yield a viable egg anyway.
