  1. Dear Dr. Sher, I read all your articles in relation to implantation dysfunction, e.g. in form of active natural killer cells caused by Thyroid problems and DQ alpha matching. I will ask my doc to do the test at the Kato clinic here in Tokyo, but wonder if they offer it here in Japan and if not where you would send the blood sample for testing, as you mentioned somewhere just knowing a handful of laboratories who could do an adequate test for both, NK cells and DQ alpha matching? Who can I contact, what are the cost and procedure if we decide to send it? many thanks in advance Yuka

  2. Dear Dr Sher,
    Can the ovary swell enough to twist after egg retrieval in patients with severe DOR producing only 2 follicles and have no cysts?
    Also what is the latest time a ovary twist can go undetected?
    I have slight discomfort and on and off nausea when not resting and it has been 1 week since retrieval.

  3. Dear dr sher , my dr , told me to have in infusion center tomorrow, 100ml of %20 iu in 250 ml saline for intralipids treatment for 90 minutes , is it sound right ?

    • It would work. I usually dilute 20% IL in 500cc saline and infuse over 3-4 hours.

  4. Hello Dr Sher,

    I would really appreciate to get your advice. I’m going through natural (no stimulation) IVF in Europe. I was told that this cycle is not ideal, so the RE will not be performing egg retrieval, however, I would like to try naturally with a trigger shot (5000 Brevactid) and intercourse.

    The clinic said to trigger tonight for timed intercourse on Saturday morning, but my concern is that this is too early? Today is CD 15 and the ultrasound reveals one dominant follicle at 15mm (it was already 15 mm two days ago, so it was having a hard time growing). E2 is 152. LH is 15.3.

    Do you please have any advice. Does it make sense to trigger tonight? Should I wait until tomorrow?
    (I wonder if the clinic triggered me too early in previous cycles since two times no egg was “found” on retrieval). Also, do you think I should use 5000 or 10,000 of Brevactid?

    • It is impossible for me to be authoritative with so little information. However bear in mind that in order to conceive, intercourse must take place before ovulation occurs.

Hi, my husband has been on steriods for 20 yrs, he’s been off them for about 2 years now, he’s also had the snip, last year he had sperm taken from him, only 2 survived, we are going for a consultation next month to see about isci, I’ve read a lot of comments that being on steriods can effect the sperm which results in a low percent that it won’t work and if it does it will also have risks to the baby? Have u any advice??? Many thanks

    • I agree that it might affect sperm production but upon stopping steroids for 3-6 months sperm function should rteurn to normal. Also, I do not believe it will increase the risk to the baby.
