  1. Dr. Sher

I have a pea sized fibroid in my uterine cavity. Ive read your blogs on fibriods and i see you believe they should be removed. However im terrified this surgery will cause scaring in my uterus that will lead to scarring and make it impossible for me to carry my own child. I already have enough problems as it is between pcos and needing immune therapy for endometriosis. Now this. What are the chances removing a pea sized fibroid will cause scarring that will make my uterus unable to allow implantation? I recently did IVF and got implantation.. I had light bleeding starting 6 days psst transfer and it went on to medium bleeding and had a biochemical pregnancy. I was treated with intralipid and demexathone prior to transfer, had great lining and high progesterone. Something went wrong but i did get implantation. Isnt that a goid sign despite? Im just terrified to have tgw fibroid removed. It would be just my luck for the surgery to make things worse. Please advise.

    • The removal hysteroscopically will NOT cause scarring of your uterine lining. Have it removed.

      Good luck!

  2. Hello Dr Sher,

I am a 37 year old women who has never been pregnant. In 2010 I lost both tubes and IVF was my only choice. I am married to a 31 year old man who does have one biological son. My AMH is .787, Follitropin is 11.0, Lutropin is 12.6, Vitamin D is 13, WBC is 4.0×10(3)mcL, RBC is 3.78x 10(6)mcL, MCV is 100.4, MCH 34.3pg, Platelets 268 x 10(3)mcL, Eosinophils is 6.7%. They also tested my C19 is 14.7, C19 Absolute 221 mcl, C16 +C56 is 6.0 and C16+C56 absolute is 90. I have a mildly disordered proliferative endometrium but they said there is not There is NO evidence of hyperplasia or malignancy.and NO evidence of endometritis.

We have done two retrievals and two transfers one frozen with a PGS tested normal embryo which failed to implant and then we did a fresh with two embryos (was on the HgH for this cycle) both failed to implant. We have no answers as to why I don’t even know what to ask my RE. My lining as been over 12+ both transfers. I have a sister and a aunt that both have Lupus and RA. My mother has pemphigus vulgaris which is a autoimmune disorder. I haven’t had an ERA done. I haven’t been tested for clotting although I have asked. I do bruise a lot without hurting myself they just appear usually all down my legs and arms. We are just lost and are looking for any information you can provide us at this time. What test should we ask to be done?

  3. Hi there! I have had two successful pregnancies (One with fertility help & one without) and my husband and i have been trying for a 3rd. I have PCOS and my LMP was 10/3/2019.
I was tracking my cycle with ovulation/preg tests to try to find out my crazy cycle length & figure out my ovulation since it’s all over the road.
I had a peek ovulation test on December 2, 2019. So I am assuming I ovulated on the third or fourth of December. I got a positive pregnancy test on the 13th. I started to bleed on the 14th and received negative pregnancy test for the next couple of days so i assumed i had a miscarriage. I saw my doctor and they agreed & they thought I had miscarried. On Tuesday the 17th I still felt pregnant so I took a test and it was positive again!! I have had blood work done and it was at first inconclusive (i believe it was so early) then 40hcg the 94hcg then 346hcg then 695 and then 46 hours later it was 1154. My doctor was worried about and ectopic pregnancy. But from what I have been reading it sounds like my numbers are doubling adequately. I am still very nervous! I have an ultrasound this Friday. I am wondering if maybe I had two embryos and lost one. This all seems so crazy to me!

    • It is possible you lost one pregnancy. Unfortunately, only time will tell!

  4. Hi Dr. Sher. Ok so here is my story. I have been doing ivf since 2012. In 2008 I had fibroid surgery myomectomy which left my uterus smaller than normal and I lost my left fallopian tube in the surgery. I did a clomid cycle in November 2009 which did result in pregnancy but it was ectopic. The ectopic surgery left me with a broken right tube that is pulled down in a V shape. 1st IVF cycle was attempted in 2012 1st transfer results in a positive but miscarried at 6 weeks. After DNC 2 weeks later I was told I had to do a saline sonogram which they found a pulp hanging down like a pendulum so my transfer was cancelled to remove the pulp. I go for pre surgical testing the next day, that evening at midnight i get a call telling me not to come in for surgery on Monday that Im pregnant. I hung up on the lady as it had to be a mistake. Me get pregnant naturally 2 weeks outside of a dnc with a broken tube but I was in fact pregnant. My daughter was born 9 months later. 3 months after I had her I found myself pregnant naturally again with my second daughter. She also was born 9 month later. At that point I had 2 frozen embryos left My husband and I decided for him to get a vasectomy which was completed in late 2014. Since 2016 or so I have been trying for one last child. Regardless of FET or Fresh transfer they wouldn’t take or I would miscarry around 6 weeks or so. My husband did the surgery to extract the sperm after being treated for months with pregyl and Letrozole i believe. Last year I did 2 cycles, first resulted in 19 eggs retrieved 0 blast to use. The second cycle 14 eggs 2 blast, One transferred results in pregnancy and miscarried again, this time we were able to send remains out for testing. It was turners syndrome. the other transfer was with a natural cycle where my egg was 19mm the day they gave me the trigger shot and they transferred 6 or 7 days after trigger failed again. The decision was made to move on to donor eggs which we did this year especially with me being 42 at the time. I purchased 6 eggs resulted in 2 blasts. 1st transfer was September this year negative beta. That cycle my E2 levels were all over the place. The did lab work 5x 1st 68 result after up to 3 estradiol pills per day then added .5 vaginally 2x per day next lab result is 668. Next test was 78 2 days later then 775 2 days after that. The day of transfer it was 137 failed transfer beta negative. Attempted the last transfer December 19. This time we used estrogen patches. 1st test results was 102, then 131 with 3 patches. They added .25 vaginally 2x per day and estrogen dropped to 78. The day of my transfer the ran the estrogen level in the office it was 147 did the transfer and Negative beta today with a 21 year old donor egg. Do you have any ideas what could be causing these failures and why my estrogen would have dropped when we added the vaginally? This is the first year that I had estrogen issues regardless of fresh or frozen. Im usually in the 2% of everything so 98% of people would go one way and my body would go the complete opposite. Im suppose to see my doctor on 01/15/2020 and i planned on suggesting redoing all labs, mutation test etc. Also redoing the ERA as it was done over 2 years ago and trying to come up with a plan to find out about the estrogen levels. Im thinking maybe im pre-menopausal. I also saw your delestrogen protocol im thinking of suggesting that as well. Any thoughts or ideas would be greatly appreciated. Thanks in advance D

  5. Hi Dr Sher, I’m curious to know your thoughts on mother breastfeeding whilst going through IVF?

I was lucky to conceive my daughter on our first round of IVF (my husband has sperm quality issues and I have no known issues and a child from a previous relationship). The cycle was a standard long protocol and we had no complications during IVF or pregnancy.

My daughter will be about 1 year old when we being our next IVF cycle and our consultant hinted that I would need to stop breastfeeding before the cycle begins, would you agree with this? Is there a reason why? I don’t want to do anything that may jeopardise our success with this next round but I also would like to breastfeed for as long as possible.

My menstrual cycles also came back 8 months after my daughter was born, in case this makes a difference.

    Many thanks
    H

    • I would definitely stop breast feeding well before your next IVF cycle and check that your blood prolactin level has dropped to normal too.

      Good luck!
