  1. Hi Dr. Sher
    I am 5 weeks 4 days pregnant. I’m having some spotting while wiping. It’s been going on for about 4 days now. Usually a peach/brown color but not enough to hit the pad. Also some red on the endometrin applicator. Not really cramping but back feels tight. Other than that I’m having no symptoms or severe cramping. Along with no pregnancy symptoms as well. I’m worried about a miscarriage. We went to my doctor and he said he sees the gestation sac still but I’m worried this is the beginning.

    • I am pretty optimistic. Do not worry about the spotting. That is very common.

  2. Hi Dr. Sher. I conceived my son thru donor egg IVF 5 years ago with the wonderful Dr. Peters at SIRM NJ. When it closed I switched practices and had two unsuccessful FET’s from the same batch of embryos. I had one natural pregnancy almost 10 years ago that ended in miscarriage. I was diagnosed with diminished ovarian reserve. Well I’m now 37 and just had the biggest shock of my life . I’m pregnant naturally. My labs are as follows so far.

    I am concerned with the rate they are increasing with my history of crappy eggs. None of mine ever survived the IVF process to transfer and when I did get pregnant on my own I miscarried. Do these look alarming for an abnormality?

    I recently had 2nd miscarriage in 4 months.
    Started miscarrying on 31st January 2020 and just had a pelvic ultrasound on 11th February 2020 (12 days from 1st day of bleeding from miscarriage).
    The ultrasound showed I only had 1 follicle in each ovary. Apparently one of them was a good size and soon to ovulate but my concern is the amount! Only 2 in total! 3 months ago after miscarriage I had 3-4 follicles in each ovary.
    Can number of follicles in this cycle be lower because I just miscarried? I still want to get pregnant. Shall I have an ultrasound during my next cycle? What cycle day would you recommend the scan to be on?

    • I would recommend an US at the start of your next period. At the same time have your ovarian reserve evaluated by measuring AMH/FSH/LH and E2 to assess how uch time you have on your side.

    • after my miscarriage during 3 months i have 2 or 3 eggs only… i waited.. after 4mounths i have 7, 8 eggs each month again

  4. Hi Dr. Sher,

    I had a PGT embryo transferred late morning on 2/2/20.

    I took a blood test early morning on 2/10/20 and received negative pregnancy results from the beta hcg.

    I’m concerned that I tested too early and received a false negative – how likely is this?

    I had been taking 1ml progesterone IM in am, endometrin (1tab) in pm, and 2 estrace in am and pm.

    • I would repeat the test in 2days! It can be a slower implantation. However, if still negative in 2 days it would be a definite -ve, in my opinion.

  5. Dear Dr Sher

    I am having a second round of IVF due to male factor issues. In my first round, I was 33 years old with an AMH of 15.5. I was prescribed 225 Meriofert per day and I stimulated for ten days.

    Eight eggs were collected, five fertilised and all five made it to day five. However only one developed into a Blastocyst good enough to transfer ( grade 4BA). My lining was 10mm (trilaminar).

    I have recently started my second round of IVF. The down regulation process began on day 3 of my period and a cyst (23mm) was observed on my left ovary at the day 21 baseline scan. My doctor was happy to begin stimulation using 300IU Meriofert per day. I have had my day 6 stimulation scan and my right ovary has 7 follicles (8-14mm) but my left ovary only has one follicle (14mm). The cyst is no longer there.

    I am wondering whether having a cyst on my left ovary may be related to the lack of follicles here? I’d be very grateful for your opinion. Thank you.

    • Hi Mairi,

      I cannot answer your question with certainty but yes…it is “possible that the cyst on the one ovary might have been a factor.
