  1. I am curious if you have ever seen numbers like these and had a positive outcome? This is a frozen 5 day transfer of 2 embryos:
    8dpt: beta 63
    10dpt: beta 127
    17dpt: beta 260
    21dpt: beta 777
my doctor says it is highly unlikely this is normal and will do an ultrasound in another week. I want to be prepared as i was already told i was miscarrying but then the hcg went up.

    • I have to agree with your RE. It does not look promising. However, besides the possibility of a failing intrauterine pregnancy, there should be careful observation and assessment for an ectopic (tubal pregnancy) also.

  2. Hi Dr. Sher,

Can you please advise how long I should wait after the birth of my 1st baby (conceived via IVF) before going ahead with another embryo transfer? I’m 37 years old and would like to optimize my best chance of success for the next transfer. Please note, I also plan on breastfeeding my 1st baby so not sure if the embryo transfer would impact from the progesterone injections.

    • After you have stopped breast feeding and have had 2 regular cycles…you wouldmbe ready6 to go!

      Good luck!

  3. Hi Dr. Sher,

My girlfriend is pregnant, She is 24. on January 6th, 2020 blood test results said 12445 HCG. Putting her with in the 6 week pregnant range.

First time I had sex with her was December 4, 2019. 4 weeks and 5 days before January 6th, 2020.

According to what the sheet the doctor said at the 4 week range her levels should be between 5-426 HCG.

    5 week range 18-7340

Her levels were 12445? Which looking at ranges for normal pregnancies puts her being 6 weeks pregnant?
Maybe like 5 weeks and 4 or 5 days?

Under more research of HcG levels 4 weeks 5 days before January 6, 2020(day of blood test) and 4weeks 5 days being the first time we had sex, if it had been my sperm that got her pregnant her levels would be somewhere around 2320, or ( 1440- 3760).

I understand It’s important to know that HCG levels will double in the first four weeks at a rate between 2-3 days. And after 6 weeks will double every 96 hours until the decline.

If you count 4 weeks from December 4th, 2019. 426 being max end at four weeks and double every 2 days you get something around 2500 HcG.

What I’m asking is is it possible that her levels could be that much higher and it be possible that that is my child. Because we are taking hormone levels around 10,000 levels higher then what would make since for it to have been my kid?

What would make her levels this much higher, I’ve only been able to find a few things online that would increase HcG levels, but from what I found it would maybe only rise the levels by about 10% not a whole extra 10,000 hormones?

Any advice and help is really appreciated because this is driving me mad not knowing and pre birth dna tests cost are ridiculous. The relationship has strained from this and she told me she’s 90% sure it is my child. I told her it didn’t matter to me but I think it actually does :/ I don’t know if that makes me a terrible partner or a band person. I really would appreciate your expert opinion so much. Thank you.

    • I understand the stress factor here but for obvious reasons, I prefer not to speculate on this very sensitive matter. However, please bear in mind that the hCG level could be markedly elevated in the event of a multiple pregnancy.

  4. Hi Dr Sher,
I am really hoping you are still answering questions. My partner and I are a full DQa match. After 2 years of trying to conceive, I have fallen pregnant naturally and am currently 7wks 3 days. My FS has put me on Dexamethezone, Clexane, Aspirin, Oripro pessaries, fortnightly 500ml intralipds, Folinic Acid tablets, Calcium and Vit D tablets. Will also be travelling to Sydney soon for 2 rounds of LIT treatments with Dr Gamal Matthias.
All of my HCG and prog levels have been tracking perfectly so far and we have seen the heartbeat several times via ultrasound.
Do you think I have any hope in carrying this pregnancy full term. Will I ever be in a ‘safe zone’, or can I miscarry at anytime? I am quite terrified and distressed and honestly just waiting for it all to end.
I look forward to hearing from you soon.

    • It all depends whether or not you also have proven NK cell activation. This is because a total DQ alpha match is only consequential if there is coexisting NK cell activation.

      Good luck!

  5. Hi Dr Sher, I was going to do a frozen donor egg transfer this month but it turns out I have three ovarian cysts, measuring 2.7cm, 2.5cm and 2cm respectively. I was told to wait until next month so the cysts could be reduced with medication (birth control.) The cysts do not give me any kind of pain or discomfort, although my cycle started on the 34th day, instead of 28th day. Should I still try this month or hold off? It’s my 6th day of menses.. I have not started any kind of medication yet to prep for IVF transfer. My estradiol level is at 720. I just wanted to get your advice on whether it was okay to proceed, knowing I have a few cysts. Will they interfere with egg transfer? Or interfere with the growth/implantation of the egg?

    • I would wait for the next cycle. Starting on day 6 of menses is a little late.
