Hello Dr Sher, I am looking for some reputable statistics on chance of pregnancy as well as chance of multiples. I am 41 years old and my husband and I had a 3 day transfer yesterday of 2 embryos. 1 was 8 cell grade 1 and the other 9 cell grade 2 (1 being best quality and 4 worst). I will add that I am an identical twin, my identical twin did IVF at age 38 and had fraternal twins. As I said in the beginning I am now 41. We’ve suffered 1 miscarriage at 7 weeks that was “natural” conception/non-IVF. I keep reading different statistics ranging from our chances of just conceiving is only @ 20% and then of that a 13% chance of multiples, on to very low chance of conceiving but if do than a higher chance of multiples bc of my age. My husband and I transferred both embryos (we had 6 retrieved and 3 fertilized, then the 2 made it to day 3). I’m not sure how many my sister/twin transferred, I know not more than 3. Any help is so appreciated, I love being a twin and adore my nephews but I am torn that maybe we should have transferred just 1 of the embryos as I would currently prefer to have a singleton. I can only give you rough statistics. I would say the chance of conception at 41Y with 2 (non-PGS tested) blastocysts transferred is under 20% (I rarely transfer cleaved embryos any longer because those that do not develop into blastocysts by day 6, are almost always chromosomally abnormal anyway). Conversely transferring 2 PGS-normal blastocysts should yield about double that success. The risk of twins with 2 X non-PGS tested embryos transferred, is likely under 10% while the transfer of 2 PGS-normal blastocysts should yield a twin rate of about 30% at age 41Y.
At 41Y of age, I would suggest a very strategic selection of an optimal protocol for ovarian stimulation and banking of PGS-normal embryos.
Older women as well as those who (regardless of age) have diminished ovarian reserve (DOR) tend to produce fewer and less “competent” eggs, the main reason for reduced IVF success in such cases. The compromised outcome is largely due to the fact that such women tend to have increased LH biological activity which often results in excessive LH-induced ovarian testosterone production which in turn can have a deleterious effect on egg/embryo “competency”.
Certain ovarian stimulation regimes either promote excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), augment LH/hCG delivered through additional administration (e.g. high dosage menotropins such as Menopur), or fail to protect against body’s own/self-produced LH (e.g. late antagonist protocols where drugs such as Ganirelix/Cetrotide/Orgalutron that are first administered 6-7 days after ovarian stimulation has commenced).
I try to avoid using such protocols/regimes (especially) in older women and those with DOR, favoring instead the use of a modified, long pituitary down-regulation protocol (the agonist/antagonist conversion protocol-A/ACP) augmented by adding supplementary human growth hormone (HGH). I further recommend Staggered IVF with embryo banking of PGS (next generation gene sequencing/NGS)-normal blastocysts in such cases. This type of approach will in my opinion, optimize the chance of a viable pregnancy per embryo transfer procedure and provide an opportunity to capitalize on whatever residual ovarian reserve and egg quality still exists, allowing the chance to “make hay while the sun still shines”.

I’m just trying to decide what to do. I have had 5 rounds of IVF. One fresh cycle was abandoned due to OHSS and a frozen cycle was abandoned due to none of them defrosting. One failed fresh cycle, one frozen cycle that resulted in pregnancy but had to be terminated as my baby had Edwards disease and my recent pregnancy of twins heartbreakingly ended at nearly 24 weeks when I went in to spontaneous labour. My daughter died within 8 hours and my son ten days later. The only reason for ICSI is my husband has a low count and motility. I have just read your page about how many times you have IVF and although I’ve had 3 proper cycles two did result in pregnancy but we were very unfortunate in what happened. So I’m just wondering if it would be worth sticking with the current treatment? Thank you.

Your story is both heart rendering and tragic. If you are a very high responder to ovarian stimulation, you need a very strategic approach to ovarian stimulation. Also, your loss at 24 weeks of twins is suggestive (but not definitively diagnastic of cervical incompetence. This also needs to be taken into account because the goal is not simply attaining a viable pregnancy but having a healthy baby.
When it comes to stimulating “high responders” who are at risk of developing OHSS,my approach is consistently to have patients launch their ovarian stimulation, coming off a monophasic birth control pill (BCP). The last few days on the BCP is accompanied by the addition of Lupron. Thereupon the BCP is stopped and Lupron therapy is continued. After 3-7 days menstruation usually ensues, at which point the dosage of Lupron is reduced and low dosage FSHr (Follistim/Gonal-F/Puregon) -dominant ovarian stimulation is commenced. Lupron and gonadotropins are then continued together. This approach is referred to as the “Long Pituitary Down-regulation protocol” Use of the BCP is intended to lower LH and thereby reduce stromal activation (hyperthecosis) in the hope of controlling ovarian androgen (mainly testosterone) release (too much ovarian testosterone is harmful to egg development). Seventy five (75) units of LH/hCG (Luveris/Menopur) is added from the 3rd day of gonadotropin stimulation. Starting on the 7th day of ovarian stimulation with gonadotropins, I start watching daily for the # and size of follicles developing and for the rise in blood [E2]. If there are > 25 follicles, the patient becomes a candidate for “prolonged coasting” I keep stimulating with gonadotropins (regardless of the [E2]) until: a) 50% of all follicles reach 14mm and b) the [E2] reaches 2500pg/ml. At that point, gonadotropin stimulation is discontinued abruptly while daily Lupron injections continue. Thereupon I follow the daily blood [E2] without doing further US examinations. The [E2] will almost invariably continue to rise. I carefully plot the rise in [E2] (regardless of how high it goes). Usually, within 1-3 days it will plateau and then start to decline. As soon as the [E2] drops below 2500pg/ml (and not before then), I administer the 10,000U hCGu (Novarel/Pregnyl/Profasi) “trigger” or 500mcg of hCGr (Ovidrel) and then schedule an egg retrieval for 36h later. ICSI is a MUST because “coasted” eggs usually have few or no surrounding cumulus cells and eggs without a cumulus layer will not readily fertilize on their own. All fertilized eggs are cultured to the blastocyst (up to 6 days) whereupon I transfer up to two into the uterus, or vitrify all expanded blastocysts for subsequent dispensation at the directive of the patient. In some cases the embryos are biopsied for PGS testing prior to being cryostored. Subsequent frozen embryo transfers are conducted as per the wishes of the patients.
It is important to point out that the success of this “prolonged coasting” approach depends on precise timing of the initiation and the conclusion of “prolonged coasting”. If you start too early, follicle growth will stop and the cycle will be lost. If you start too late, you will encounter too many post-mature/cystic follicles (>22mm) that usually harbor abnormally developed eggs.
Use of the above approach avoids unnecessary cycle cancellation, severe OHSS, and optimizes egg/embryo quality. The worst you will encounter is mild to moderate OHSS and this too is uncommon.

We have been trying to conceive for 2 years. Every month I get brown spotting for approx a week during the luteal phase. I’ve had various tests and treatments which haven’t fixed this. Surely this is the lining coming away too soon making implantation impossible. I’ve tried 1200 mg of cyclogest pessaries/and Lubion injections but I still spot. Will this be contributing to our infertility?
Also I take a daily baby aspirin but since taking it I ovulate late or have LUF cycles could this be the cause?

It sounds as if you have a dysfunctional ovulatory problem, which could be a luteal phase insufficiency. However, I would need a lot more information to know one way or the other definitively. There could aso be a surface lesion (such as a uterine polyp) in the uterus. In the absence of the latter it is likely that ovulation induction with fertility drugs could resolve that issue.
Following ovulation, the remainder of the dominant follicle transforms itself into a structure called the Corpus Luteum (CL) which produces progesterone. The purpose of progesterone is to prepare the uterus to accept and support an early pregnancy until it is able to sustain itself at around 8-10 weeks of gestation.
The life span of the CL is predetermined to be 12-13 days, unless rescued by a signal from the early pregnancy. If no pregnancy occurs, the CL stops making progesterone and menses ensues 1-2 days later. For most women the length of the second half of the menstrual cycle (the luteal phase) is constant at 14 days. A small percentage of infertile women (3-4%) have a shortened luteal phase. This may result in the loss of pregnancy support before the budding pregnancy has a chance to signal the ovary that it is there.
The lining of the uterus (the endometrium) has a specific appearance that changes throughout the menstrual cycle, such that a biopsy of the lining a few days prior to expected menstruation, can accurately date endometrial development. A 3 or more day difference between endometrial dating by biopsy and cycle day as determined by the start of the next menstrual period is indicative of a luteal phase defect (LPD). Sequential mid luteal progesterone levels < 10 ng/dl can also be used to diagnose a LPD. Luteal phase defect can be treated with Clomiphene Citrate, Progesterone supplementation or hCG injection PROGESTERONE LEVELS Normal Progesterone (P) levels rise sharply after ovulation, peaking a week after ovulation. Because P secretion occurs in a pulsatile fashion (around every 90 minutes), single low levels are often found in the course of a normal luteal phase and cannot be used as a predictor of luteal defect or poor fertility potential. In pregnancy, P is secreted by the corpus luteum (of pregnancy) until about 10 weeks of gestation. Because of the large variation in individual P levels, the predictive value of single P measurement in early pregnancy is limited. Usually levels above 20 ng/ml indicate a normal pregnancy while levels below 5 ng/ml suggest an abnormal pregnancy (ectopic or miscarriage). It is also important to point that persistent low P levels are usually the result, rather than the cause, of poor outcome and that unfortunately, as a result, P supplementation does not prevent the inevitable. I have had my progesterone levels tested and they are always good. I have done cycles with clomid and extra progesterone where my levels when tested were over 100 yet I was still spotting. I had a hysteroscopy last year which found nothing untoward inside my uterus so I am baffled and frustrated!
We just failed our first IVF cycle but have 5 blastocysts frozen so we are about to try again and the clinic are adding in Lubion injections along with progesterone pessaries. Im just anxious that something is causing this spotting and will prevent pregnancy. But we have spent thousands seeing various specialists and cannot get to the bottom of it. I have pcos which I guess could be a factor. But even on clomid and progesterone support I still spot.
I feel like we will never conceive with this issue.
Interestingly last month I had a long cycle which I’m told was due to a cyst (had a scan) and I had no spotting at all!
I have stopped the daily baby aspirin in the hope that I will ovulate more regularly

I would like to use my own eggs. Does medication prior to IVF increase chances of pregnancy? Is IVF the best option for those with low AMH?

ICSI is advisable but not a must and yes…especially for women with diminished ovarian reserve, fertility drugs are important to launch the cycle.
