I’ve been on estrogen patches weaning off today. Started progesterone suppository TID yesterday.

Hello,
We have been trying for a baby for a while and hadn’t been successful and so we underwent initial fertility tests and we found out that my husband’s sperm morphology and motility were well below where they needed to be. The doctor told him to take proxeed which he has been doing but we are now both considering taking Impryl. We have been trying to compare Proxeed and Impryl and they seem to focus on two different things (for men) and so we are not sure whether he can take both or whether he should stick with one, (we will have a further sperm test in a couple of months) please do you have any advice?
Is there any reason why a woman shouldn’t take Impryl? As far as we know I have no fertility issues but I do have PCO that results in slightly irregular periods.

Hi Dr. Sher,
I have PCOS, and this is my first pregnancy achieved through IUI.
I’ve had cramping and slight back pain since the day of my IUI, but no bleeding. I’m currently taking 400mg progesterone suppositories and 50 mcg levothyroxine.
My HCG levels are unusual, according to the nurse, and the doctor is concerned. An ultrasound will be performed at 25dpo to rule out an ectopic pregnancy. What are your thoughts on the matter?

I would like to ask about testosteron gel pretreatment in ivf for poor responders.
When I was using 25 mg dosage on a daily badis for 6 weeks before stimulation I got great results. Now that I am back on treatment I wonder what is the minimal restosteron dosage required for restoring the overian respond. Thanks a lot, Irit

I recognize that there are different opinions on this topic. However, I am vehemently against testosterone supplementation, especially when it comes to older women and those with diminished ovarian reserve.
Consider the following: Ovarian testosterone is needed for follicular development. However, the amount required is small. Too much ovarian testosterone spills over into the follicular fluid can have a deleterious effect on egg/follicle development. Some women (women with diminished ovarian reserve –DOR, older women and those with polycystic ovarian syndrome-PCOS) who tend to have increased LH biological activity, already over-produce testosterone. To such women, the administration of testosterone to such women, by “adding fuel to the fire” can be decidedly prejudicial, in my opinion. Young women with normal ovarian reserve do not over produce LH-induced ovarian testosterone, and are thus probably not at a comparable risk.

Hi Dr. Sher! Congratulations on your announcement today. Quick question about hcg and progesterone.
My hcg draw was at 15 dpo 300 and 48 hours later it was 832. My question is- im going to get another draw. Is it better that the rise continues the same trend of rising the way it did? For instance, my number more than doubled a bit. So with the next draw should it also more than double? Or if it only doubles, is that a sign of a problem? And my second question is with progesterone- it was 26.7 and rose to 28 in those two draws. Should it keep rising? If it doesnt is that a sign of a problem?

While it wont do harm, I do not think there is a need for another hCG test. Rather I would wait to 7 weeks and do an US. In my opinion your progesterone level is fine!
