  1. Hi. I’ve been reading your protocol for FET and have a few questions. This is my last attempt at a baby due to age and financial abilities, so I want to do everything to make this work.
    I take Intralipids for NK cells. Will this also help for endometriosis?
    I have no polyps and my tubes are open and fine.
    According to your FET protocol, Dexamethasone is supposed to start when Lupron starts. I started Lupron on 12/14/21, but don’t start dex until 1/8/22. Should I go ahead and start the dex today? Will this delay in dex cause a miscarry?
    Also, there isn’t any antibiotic on my calendar like Cipro. Should I start this as well according to your protocol? My transfer is 1/14/22.
    I also do not have any vaginal medicine on my calendar—should I ask for this?
    My pre-lining check today was 9.46. This was based on .2ml of estradiol and now I’m to start .35ml of it.
    I trust your recommendations to the fullest so I would like to know what you think would be best at this point in my FET cycle concerning these meds. Thank you.

    • As long as you start the dexamethasone with the initiation of estrogen therapy for a medicated FET, you should be fine. . A short delay should not increase the risk of miscarriage. As for use of Cipro…discuss with your own RE.
      All this should be discussed with your treating RE.

  2. Dear Dr Sher,
    I have been treated for Asherman syndrome. However, my lining remains very thin and never grows past 6.5mm even with medication. Although my lining once grew to 11 mm when I developed an ovarian cyst. I have had a failed IVF cycle already and I don’t want to try IVF again if I can never get successful implantation. I have triple lining and my dr thinks that’s reason enough to keep trying. What do you think? What do you think about PRP intrauterine rejuvenation?

      My period did not return for 8 months after a D&C. I was finally diagnosed with intrauterine adhesions and 2 hysteroscopy procedures to remove scaring. My period returned but was very light. lasted 3 days, compared to 6 days before my D&C.
      After trying to get pregnant on my own for 6 months, I did 3 cycles of letrozole. I always had 3-4 large follicles but never got pregnant. Then we decided to go for IVF; I had 5 embryos on day 5, uterine lining was 6.5mm. I had a positive pregnancy test on day 12 with Beta at 95 but the number declined by day 16. Although my lining did not grow much with Estrogen pills, 2mg, 3x a day, I once developed an ovarian cyst after my failed IVF and my lining grew to 11mm. But once my cyst was gone, my lining was back to being very thin.
      My prolactin level was at 129ng/ml and I was put on cabergoline…0.5mg 2x a week. (TMI)>>>>My period now only flows for 2 days, and it’s very dark and thick. My doctor wants me to go for another round of IVF, but I fear it may be a waste of money as certain underlying issues have not been addressed. My AMH is 2.4, my HSG looks normal and hysteroscopicc view of my uterus also looks fine, thyroid test results are also normal. I don’t know how to proceed. The fact that my period hardly flows and remains dark all through leads me to believe there’s more to be addressed.

  3. Transferred 2 laser hatching embroys on 16-12-21 and beta HCG on 30-12-21 is 46. After 2 days it was 81. Doctor said to repeat test after one week again. Please suggest me. I am so worried.

    • It is a slowish rise but I am guardedly optimistic for you. I am afraid, only time will tell!

  5. Dear Dr Sher,

    I did a natural cycle FET just over two weeks ago, and had a positive beta of 480 at 11/12 days past the transfer.

    My progesterone level the day before transfer was 57nmol, and on the day of the transfer it was measured as 47nmol so a few days after I was asked to start taking 400mg cyclogest once a day. I started this after implantation would have already happened.

    At my beta my progesterone was 97nmol. My clinic originally advised me to stay on it, but I contested this because it’s causing me lots of irritation and I worry about giving the foetus unnecessary hormones when I have a corpus luteum. I think my literal phase is about 12 days, so on the shorter side perhaps.

    Im now really torn, I desperately want to come off the progesterone but I’m really worried this will induce a miscarriage. My clinic aren’t being that helpful and just told me to get blood tests elsewhere send them results to check levels are still fine.

    Please can I ask what you suggest?

    • I would follow the advice of your treating RE. However, ask him if you can switch to Vaginal progesterone (Crinone 8%).
