Dear Dr. Sher,
I am 38 years old with a FSH (done on day 3) of 7.9 U/L. I have an endometrioma inside my left ovary that is measuring about 5cm.
I am desparate to have a baby. In your opinion should I have laproscopy and get the endometrioma out or go straight to IVF? I am worried that with the laproscopy I will diminish the function of my left ovary and reduce my overall chances of conceiving.
Women, who have advanced endometriosis, often have endometriotic ovarian cysts, known as endometriomas. These cysts contain decomposed menstrual blood that looks like melted chocolate…hence the name “chocolate cysts”. These space occupying lesions can cause chronic pelvic pain, pain with intercourse (dyspareunia) and painful menstrual periods, thus compromising quality of life. They can also activate ovarian connective tissue (stroma or theca) resulting in an overproduction of male hormones (especially testosterone). An excess of ovarian testosterone can severely compromise follicle and egg development in the affected ovary. Thus there are two reasons for treating endometriomas. The first is to alleviate symptoms and the second is to optimize egg and embryo quality.
Conventional treatment of endometriomas involves surgical drainage of the cyst contents with subsequent removal of the cyst wall (usually by laparoscopy).Unfortunately, with surgery normal ovarian tissue can be inadvertently removed/damaged along with the cyst wall, thereby decreasing the number of available eggs for harvest. Since many women who have endometriomas also have advanced endometriosis and have had one or more previous surgeries, they often .have significant scarring and adhesions. This can compromise visualization of and access to anatomic structures during conventional laparoscopic surgical correction, increasing the risk of surgical complications.
Many patients with recurrent ovarian endometriomas are uncomfortable with the prospect of repeat surgery and its avoidance is often a factor in their decision to proceed with IVF. We recently reported on a new, effective and safe outpatient approach to treating endometriomas in women planning to undergo IVF. We termed the treatment ovarian Sclerotherapy. The process involves; needle aspiration of the “chocolate colored liquid content of the endometriotic cyst, followed by the injection of 5% tetracycline hydrochloride into the cyst cavity. Such treatment will, more than 75% of the time result in disappearance of the lesion within 6-8 weeks. In some cases the injection of tetracycline into the endometrioma causes a reaction that results in clear or blood stained fluid collecting in the original cyst cavity where the endometrioma had been. Upon re-aspirating the fluid in the seroma, the lesion will usually disappears permanently most times. In a small number of cases, the endometrioma comes back and sclerotherapy must be repeated or surgical removal undertaken.
Ovarian sclerotherapy can be performed under local anesthesia or under conscious sedation. It has the advantage of being an ambulatory in-office procedure, low cost, and has a low incidence of significant post-procedural pain or complications and the avoidance of the need for laparoscopy or laparotomy.
Sclerotherapy is a safe and effective alternative to surgery for definitive treatment of recurrent ovarian endometriomas in a select group of patients planning to undergo IVF. Since the procedure is associated with a small, but yet realistic possibility of adhesion formation; its use should be confined to cases where IVF is the only treatment available to the patient. Women who intend to try and conceive through fertilization in their fallopian tubes (e.g; following natural conception or intrauterine insemination) are better off undergoing laparotomy or laparoscopy for the treatment of endometriomas.

I’m an IVF patient in Australia. One of my friends visited you for assistance after many years of failures and she now has a beautiful baby.
I have had two rounds of IVF with no success both antagonist. First round I was on Elonva (100)/Puregon (300)/Orgalutran (250). I had 11 follicles, 5 eggs, all fertilized but all cleave arrested day 5. Second round I was on Gonal F (300)/Menopur (150)/Orgalutran (250) with 5 follicles, 2 eggs, 1 fertilized and transfer at day 2 with negative pregnancy.
I’ve now been on a down regulation cycle and am on Gonal F (450)/Luveris (75)/Syneral. Plus steroids (10mg) as I have high natural killer cells >30.
At my second scan today, I only had three follicles with only one at correct size. My epu was scheduled for early next week but now may be cancelled.
I seem to be labeled a ‘poor responder’ with most likely poor quality. Any chance for me to increase my egg numbers? I’ve just turned 38 and my AMH is 8.1. Is there a particular drug protocol I could explore?

Hi Dr Sher, is it possible to tell whether it is the sperm or egg that causes chromosomal abnormalities? I know it is generally considered to be the egg that is the key factor but how can one tell that it is not the sperm?
If repeatedly all embryos are turning out to be chromosomally aneuploid then you could try: doing a polar body biopsy on the egg in order to exclude egg aneuploidy or (less reliably) ,fertilize some of the the eggs with husband’s sperm some with donor sperm and then test do PGS on resulting embryos.

I suffered from secondary infertility for ~2 years.
After conceiving naturally and gave birth via C-section. Afterwards, I experienced several chemical pregnancies. I was treated for thin lining & was finally diagnosed with Ashermans. My new RE reduced scarring (top left corner) from 30 to 10% in May.
We then did a round of IVF.
Only stimmed for 7 days before trigger. Lupron Microdose: 17 eggs retrieved, 8 mature, 4 fertilized, 3 3-day blasta, 1 5-day blast. (PGS test= chromosomally normal with 95% confidence). We haven’t attempted to transfer the embryo.
I’m turning 40 in 2 weeks. I think I have PCO (some cysts on ovaries, ovulate regularly, but with 15/16 day follicular phase). Taking metformin.
Questions:
1.If we did another round of IVF, which protocol would you recommend? (RE is recommending Antagonist this time).
2.How important do you think it is to remove the remaining 10% scar tissue before doing a transfer?

If there is scarring of the uterus this shoul,d be removed. However, ultimately it will also depend on endometrial thickness.In 1989, I first demonstrated that in both normal and “hormonally stimulated” cycles, preovulatory endometrial thickness as assessed by ultrasound examination, is partially predictive of embryo implantation (pregnancy) potential following IVF. Ideally the endometrium should measure at least 8.0mm in thickness, (but preferably >9mm). A “poor” endometrial lining is most commonly due to: 1) inflammation of the uterine lining (endometritis) that usually occurs as a result of endometritis (inflammation of the uterine lining that can follow a septic delivery, partial retention of the placenta following delivery, abortion or miscarriage, 2) severe adenomyosis (gross invasion of the uterine muscle by endometrial glandular tissue), 3) multiple fibroid tumors of the uterine wall) 4) prenatal exposure to the synthetic hormone, diethylstilbestrol (DES) and, 5) following >3, consecutive, back to back cycles of clomiphene citrate ovulation induction.
Treatment with vaginal Sildenafil (Viagra): Hitherto, attempts to augment endometrial growth in women with poor endometrial linings by bolstering circulating estrogen blood levels (through the administration of increased doses of fertility drugs, aspirin administration and with supplementary estrogen therapy) have yielded disappointing results.
In the mid-90’s I first reported on the finding that thee vaginal administration of Viagra for several days prior to the “hCG trigger “ or progesterone administration enhances uterine blood flow and estrogen delivery to the uterine lining and so improves endometrial thickening. Then In October 2002, I reported on the administration of vaginal Viagra to 105 women with repeated IVF failure due to persistently thin endometrial linings. All of the women had experienced at least two (2) prior IVF failures attributed to intractably thin uterine linings. About 70% of these women responded to treatment with Viagra suppositories with a marked improvement in endometrial thickness and 45% of these women achieved live IVF- births following a single cycle of treatment with Viagra. Nine percent (9%) miscarried. None of the women who had failed to achieve an improvement in endometrial thickness following Viagra therapy, subsequently and who underwent embryo transfers achieved viable pregnancies.
The older a woman becomes, the more likely it is that her eggs will be chromosomally/genetically “incompetent” (not have the potential upon being fertilized and transferred, to result in a viable pregnancy). That is why, the likelihood of failure to conceive, miscarrying and of giving birth to a chromosomally defective child (e.g. with Down Syndrome) increases with the woman’s advancing age. In addition, as women age beyond 35Y there is commonly a progressive diminution in the number of eggs left in the ovaries, i.e. diminished ovarian reserve (DOR). So it is that older women as well as those who (regardless of age) have DOR have a reduced potential for IVF success. Much of this is due to the fact that such women tend to have increased production of LH biological activity which can result in excessive LH-induced ovarian male hormone (predominantly testosterone) production which in turn can have a deleterious effect on egg/embryo “competency”.
While it is presently not possible by any means, to reverse the age-related effect on the woman’s “biological clock, certain ovarian stimulation regimes, by promoting excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), can make matters worse. Similarly, the amount/dosage of certain fertility drugs that contain LH/hCG (e.g. Menopur) can have a negative effect on the development of the eggs of older women and those who have DOR and should be limited.
I try to avoid using such protocols/regimes (especially) in older women and those with DOR, favoring instead the use of the agonist/antagonist conversion protocol (A/ACP), a modified, long pituitary down-regulation regime, augmented by adding supplementary human growth hormone (HGH). I further recommend that such women be offered access to embryo banking of PGS (next generation gene sequencing/NGS)-selected normal blastocysts, the subsequent selective transfer of which by allowing them to to capitalize on whatever residual ovarian reserve and egg quality might still exist and thereby “make hay while the sun still shines” could significantly enhance the opportunity to achieve a viable pregnancy
