Hi Dr Sher,
We had our first failed IVF cycle last month. Two grade A blastocysts were transferred. It was a difficult transfer and took two attempts, the doctor thinks this could be the reason it failed. The thing that’s bothering me is that my endometrial lining was 20.6, which is obviously incredibly thick. Do you think this could be a contributing reason? And if so, what could be done about it? I’m 37 with tubal factors (no hydrosalpinx).

Hi Jessica,
There is no such thing in my opinion as an overthickened endometrium.As long as this thickened endometrium is not due to endometrial pathology such as hyperplasia, or (unlikely) malignancy, this would in my opinion not adversely affect implantation.
So Your RE is probably correct. The ET process is a very important one.

Hi,
I was supposed to start my 2nd round of ivf stims tonight. At my baseline ultrasound showed 21mm follicle and my estrogen was 261. They said it was up to me if I wanted to continue or cancel this cycle. What should I do? Also I was taking B.C pills for month to suppress but obviously didn’t work. Could I become pregnant naturally if I just stopped B.C pills yesterday?

Dr. Sher I have just completed a long protocol recommeded for people like me with pcos but the coasting lasted 6 days . I had over 40 follicles but only got 6 mature eggs I was Devasted. .. 6 follicles were over 22mm and 16 in between 14 and 22 mm… why did this happen in your opinion. Did I coast for too long?
Seems w each ultrasound the embro growth is consistent w being approx4/5 days behind , therefore I’m wondering if it can be just slow growth or late implantation or my wishful thinking . I am leaning towards waiting another full week before scheduling a D&C ( approx 12 days from today vs 5 days from today) pls comment on ur thoughts
If the conceptus is growing and there is HB. Why would you do a D&C. Give it more time.

Dear Dr, On Jan 7th I xfered 4 embryos @ 3days. 1@ 8cell,2@ 7cell and 1@ 6cell.
Jan20th positive Pg 519 HCG level.#1 Us on jan26 – 3mm gest sac 3100HCG Prg/Est all in range .#2US -Feb 4 fetal pole 3.13mm. Hcg +5000k ,prog and Est all in range.
#3 Today I had my follow Us – fetal pole- no heart beat. HCg level over 5k, progesterone and est are all in range
D&C scheduled next wed feb 17.
My questions is – can timing be off? late implantation ? last ultrasound will be day of the D&C before procedure, is that giving myself enough time for a heartbeat to show up?
should I wait a bit longer before I do a D&C. is it possible that this is just growing slowly? should I wait till 9 weeks to know for sure there is no heartbeat? ? is giving myself till next Wednesday adequate time period to make a def decision on d&C.. ? I’m scared that I’m being pre mature with this decision , drs recommend D&C. Today I measured over 5mm I’m unsure as I was in a daze after hearing about D&C to pay much attention but Quick glimpse I saw over 5 mm CRL I have had no bleeding/spotting thus far. ..im at a crossroads.pls provide your advice – be very grateful.
By my calculation you are now >7weeks and there should be a HB by now. I do not think this is a viable pregnancy. However, there is no harm giving it another week, if only for your peace of mind.

I was wondering if there are any instances where you recommend a higher dose than 0.75mg of dex for autoimmune (w/o alloimune) ID? In instances of repeated IVF failure despite good embryos with immune protocol? Or is this dose always consistent regardless? Thank you
