  1. I went through my first cycle of IVF this past June. Everything was going well until I gave myself the trigger shot of Novarel that night. When I went in for my egg retrieval when I woke up the doctor said my eggs were gone. He really didn’t have a response for me other that he was sorry and that I’m a poor responder to the medication. I’m 40 now and everything has went south since then. The doctor since has said that I need a donor egg which is out of the question. I was wondering if you had ever heard of anyone having an allergic reaction to this medicine?

    • You either ovulated prior to the procedure or you had “premature luteinization . There is no such thing as empty follicles (see below). This all likely has to do with the protocol used for ovarian stimulation….see below!

      The older a woman becomes, the more likely it is that her eggs will be chromosomally/genetically “incompetent” (not have the potential upon being fertilized and transferred, to result in a viable pregnancy). That is why, the likelihood of failure to conceive, miscarrying and of giving birth to a chromosomally defective child (e.g. with Down Syndrome) increases with the woman’s advancing age. In addition, as women age beyond 35Y there is commonly a progressive diminution in the number of eggs left in the ovaries, i.e. diminished ovarian reserve (DOR). So it is that older women as well as those who (regardless of age) have DOR have a reduced potential for IVF success. Much of this is due to the fact that such women tend to have increased production of LH biological activity which can result in excessive LH-induced ovarian male hormone (predominantly testosterone) production which in turn can have a deleterious effect on egg/embryo “competency”.
      While it is presently not possible by any means, to reverse the age-related effect on the woman’s “biological clock, certain ovarian stimulation regimes, by promoting excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), can make matters worse. Similarly, the amount/dosage of certain fertility drugs that contain LH/hCG (e.g. Menopur) can have a negative effect on the development of the eggs of older women and those who have DOR and should be limited.
      I try to avoid using such protocols/regimes (especially) in older women and those with DOR, favoring instead the use of the agonist/antagonist conversion protocol (A/ACP), a modified, long pituitary down-regulation regime, augmented by adding supplementary human growth hormone (HGH). I further recommend that such women be offered access to embryo banking of PGS (next generation gene sequencing/NGS)-selected normal blastocysts, the subsequent selective transfer of which by allowing them to to capitalize on whatever residual ovarian reserve and egg quality might still exist and thereby “make hay while the sun still shines” could significantly enhance the opportunity to achieve a viable pregnancy

  2. Hi I am 38 and have had 2 failed IVF cycles using ICSI. I had a child naturally and with no problems getting pregnant 4 years ago. I am now on my own, hence doing IVF with a donor. I have low AMH, low but not as low as originally thought. Both cycles were long down regulation using dubstep for down regulation, first cycle was with 400iu of Gonal F and stimulated for 14days, 250 of ovidrel for trigger I got 7 eggs 5 fertilised and all were top quality on day 3 at day 5 all but 2 had arrested one was at morula stage and one was further behind this. No success and period arrived before pregnancy testing date. On my second cycle I spoke to my FS about concern of too much FSH affecting the quality of my eggs so this time we started with 250iu of Gonal F after I had 2 leading follicles and 6 smaller ones after about 7 or 8 days of stimulation my FS said we needed to increase to 400iu of gonal f as weren’t getting enough follicle/hormone response, so increased this and stimulated for another about 6 days (14 days all up). 2 follicles were still bigger than others, about 3-4medium and 2 small. This time we triggered with 500 of ovidrel as my FS thought eggs may not have been mature enough first cycle. EPU is done mon, wed, fri in this clinic. So last hormone monitoring was done on Friday morning and told to trigger sat evening for Monday morning EPU. Come time for EPU and I had already ovulated, so only retrieved 3 eggs 2 I were told were mature one not. Both fertilised with ICSI and high digital magnification sperm selection. Both were grade A day 3 embryos (one was even starting compaction on afternoon of day 3 which is when we transferred both back due to only having 2 embryos, used assisted hatching and embryo glue and again not successful. My questions is what do you think is going wrong? What protocol would you use for me? And why did I ovulate before egg pick up? Was this poor monitoring, I questioned the larger size of the 2 biggest follicles and whether they would be at risk of being ‘too big/lost’ and my FS assured me my hormone levels were fine and this wouldn’t happen? If I do another cycle what should we be done Ng differently? Thank you for your help

    • You clearly have diminishing ovarian reserve. It is essential that oin such cases the protocol used for ovarian stimulation be carefully constructed and implemented. Also, in my opinion, 250mcg Ovid]rel is too low a trigger dosage. You would need 500mcg or 10,000U Pregnyl/Profasi or Novarel.

      Women who (regardless of age) have DOR have a reduced potential for IVF success.   4. Hi Dr. Sher.
    I am planning on doing a frozen embryo transfer. I have only one chromosomally tested normal embryo. I am thinking about doing an embryo scratch test prior to doing the frozen embryo transfer. When is the right time to do the scratch test before the frozen embryo transfer. I have read that if it is done too close to the frozen embryo transfer it can lower the pregnancy rates. Thank you.

  5. Dear Dr
    I have had 1 miscarriage at 9 weeks (no heartbeat found) and 1 biochemical pregnancy. Later after taking so many tests it was found that I have abnormal balanced chromosome. Then we opted for PGS but all the embryos were abnormal. So we went for donor eggs.. got only 3 embryos from her.. transferred twice but both ended up negative.. then again we went for ivf with donor eggs.. this time we have got 12 embryos. Transferred 2 at first but again negative.. then again we transferred 3 embryos and this time I got pregnant.. this is my 8th week.. doctor suggested Down syndrome screening and NT scan on 12th week.. I am really scared.. is it still possible tat my baby might have any chromosome abnormality or Down syndrome even after using donor eggs?? Please reply me.. my age is 26yrs and my husband’s age is 29.. got married in 2013.. in treatment since January 2015..
