Hi Dr. Sher. I am 33 years old. My Day 3 FSH has come back between 10 and 16 every time it’s been tested within the last year. My AMH was 3.2 ng/ml and my antral follicle count was 5 total. I have attempted 3 IVF cycles, 1st one was cancelled because of a 19mm cyst, 2nd cycle cancelled because of high FSH of 16 and cyst, 3rd attempt we proceeded with a FSH of 10 and aspirated my cyst. I was on an estrogen primed antagonist cycle with 7 days of 4 mg Estrace daily around day 21, and then started on day 3 with 11 days of Gonal F 450 and 150 Luveris. Added in 250 Centrotide from day 8 to 12. I ended up getting converted to an IUI because my 5 follicles were 20,21,13,12,9 and my E2 was 2578 on day 11. The IUI was unsuccessful. My clinic only does egg retrieval with a minimum of 4 mature follicles.
I have been told donor eggs is my best chance of a baby. What are your thoughts on my protocol and do you have a recommendation if I should try IVF again with my own eggs. Is there another protocol you would suggest? I have been deemed a poor responder, am confused with my high FSH and relatively normal AMH result. Thank you for your help 🙂

Hello Dr. Sher. I’ve been reading your blog(s) a lot lately. It is a great resource of fertility related information for laypeople. Thank you very very much for that, I learned a lot from you. I wonder if you can answer a question of mine. I recently read about this very fact – that BCP, as well as estrogen and progesterone, can interfere with the development of those preantral follicles into the antral ones in the days leading up to menstruation. Does this then mean that if in one cycle the progesterone is taken (say Utrogestan 100 mg 2×1 daily) in the luteal phase, then the following cycle the antral follicle count will be smaller and thus the possible IVF in the following cycle will likely start out with fewer follicles which then, down the road results in fewer eggs retrieved and embryos grown. And if you recommend to counteract the effect of BCP by the action of an agonist would the same work with the effect of progesterone? I would very much appreciate your answer.
The reason I ask is that my AFC has dropped markedly from around 10 on only one healthy ovary (the other one with a big cyst) to only 2-4 per ovary in the span of a few months. I wonder if it could be because of the use of Primolut-Nor in the ten days following the cyst drainage and Utrogestan in the following cycle’s luteal phase?
The BCP will not reduce the # of antral follicles, in my opinion. It is possible that the use of Primolut-Nor in the ten days following the cyst drainage and Utrogestan in the following cycle’s luteal phase could have played a role in your blunted response.

Dear Dr. Sher,
I would just like to ask for your opinion. How long should a patient (under hormone replacement and 2 day 5 blastocysts were transferred) wait after fet to know if she’s pregnant or not?
My doctor asked me to come back 5dpt to check if I am pregnant or not. Isn’t it too fast? I have always heard of the 2ww. I am a little surprised. Please enlighten me.
I recommend 7 and then again 9 days post ET.

hi Dr Geoffsher, please i and my husband has do all the necessary test as regard conception but we are unable to produced any child for 10 years now…i have never conceived for once…is there any possibility that antibody anti-sperm test could be an option in my cause..and if it appears positive..is there any treatment yet for treating such case..help me..i am 41years of age and my husband is 44years of age..we are from west Africa(Nigeria)

Hello Dr Sher. I was due to have a frozen ET this month but I started bleeding mid cycle. A blood clot was discovered on u/s scan and removed via hysteroscopy. Inspired by your website, I’d just started using vaginal viagra, could this have caused it? I have no access to the vaginal preparations so I was just using the pills designed for oral usage. No! It would not have caused this. BUT using the pill is not effective. You need a speciqally prepaired compounded Viagra vaginal preparation.
