I am a 38 year old female diagnosed with PCOS. My Husband’s age is 41 years old. I have history of right ovarian torsion and right tubal ectopic pregnancy (naturally conceived). In the last surgery for ovarian torsion release the doctor had to cut off right tube and partially excised my right ovary. Now we are undergoing our first cycle of IVF. Our doctor opted for mild stimulation multiple cycles. As I have completed one cycle, it was seen that my right ovary did not respond at all. From my left ovary there were 5 Follicles of which 2 eggs were retrieved. Both the eggs fertilised normally a were frozen on day 3 with 8 and 9 cells respectively. Fragmentation was 5% and 10%. What does that mean? Why did the doctor not culture the embryos upto blastocyst?

Dear Dr. Sher,
I am 33 (34 in January) and my husband is 34 with two recent failed IVF attempts, and I’m hoping to get your advice on the protocol for my next cycle (my RE is suggesting estrogen priming) and any additional steps we might be able to take.
Some background on me and my husband…We first started to try to get pregnant 5 years ago, and after almost a year of trying I was diagnosed with “lean type” PCOS based on irregular periods, elevated bloodwork, and “string of pearls” on ultrasound. We had several failed pregnancy attempts with first Clomid then Femara. However, about 2 years ago, my periods suddenly started to become very regular, and I fell pregnant naturally in the summer of 2015, but unfortunately miscarried in September (about 8-9 weeks). According to my current RE, I do not seem to have PCOS at all based on bloodwork and ultrasound. My husband has always had normal semen analysis results, but he did have a variocele repair several years ago.
With our first IVF attempt in September, I was started on BCP for about 2 weeks and then stimmed with Follistim and low dose HCG. Ganirelix was added later in the cycle. Because of high estrogen levels and a large number of follicles, my RE did just a Lupron trigger. At retrieval, they got 16 eggs (15 mature). 9 fertilized with ICSI, and 3 made it to day 5/6 for PGS. Only 1 embryo came back as normal, and our medicated FET cycle failed the next month.
My RE used a similar protocol the next cycle with similar results. She did increase my dosages of Follistim and low dose HCG (hoping to get more eggs) and used both Lupron and HCG for the trigger. This time we got 15 eggs (all mature), 10 fertilized with ICSI, but only 1 made it to day 5/6 for PGS. I just found out yesterday that it came back abnormal.
For the next cycle, my RE is going to have me take 2 mg of estrogen beginning after ovulation, but then use the same meds (Follistim and low dose HCG) for stimulation. This sounds like a good plan to hopefully improve egg quality, but is there anything else we can do to help? My husband did start taking antioxidants (including Vitamin C and E) in case DNA fragmentation was the culprit.

Thus while I personally do NOT use estrogen priming on women with high or normal ovarian reserve (e.g. PCOS), some might do so. In my opinion, there is little benefit in doing so and again, in my opinion, further bolstering of the estrogen level might not good for the eggs in such cases.

Me,33, AMH-1.2, Obese- BMI 38, but regular cycles, no PCOD, clean SHG and HSG, patented tubes, ovulate on my own every cycle on the dot. Hormones TSH, Lh, FSH, all normal as per treating doctor.
Husband has low sperm count 1.8 million/ml during last IVF done in March 2016, he had a varicocelectomy done earlier in 2013. We are trying to conceive since 2011.
Already tried 6 cycles of clomid ovulation induction , 4 cycles of IUI , 1 Long Protocol IVF , and a FET with no success. 1st IVF had 9 eggs retrieved, 4 made it to blastocysts and 1 was transferred, had a chemical pregnancy wiht beta value 12 2 weeks after transfer;Doctor also hinted that it could be auto immune issue for the 1st ivf failure; 3 frozen embryos were transferred during FET no success. Now we are on Antagon protocol with growth hormone administered for days… cetrotide used as antagon.. right now doc says the response was less than the previous IVF, only 2 or 3 follicles are growing uniformly and 3 are lagging behind.. We plan to complete this anatagon cycle but am worried about the results. What are my chances of conceiving with my own eggs? what protocol works best for me? am i on the right track? I want to conceive with my own eggs, please let me know what i need to do to achieve the same.

Whenever a patient fails to achieve a viable pregnancy following embryo transfer (ET), the first question asked is why! Was it simply due to, bad luck?, How likely is the failure to recur in future attempts and what can be done differently, to avoid it happening next time?.
It is an indisputable fact that any IVF procedure is at least as likely to fail as it is to succeed. Thus when it comes to outcome, luck is an undeniable factor. Notwithstanding, it is incumbent upon the treating physician to carefully consider and address the causes of IVF failure before proceeding to another attempt:
1.Age: The chance of a woman under 35Y of age having a baby per embryo transfer is about 35-40%. From there it declines progressively to under 5% by the time she reaches her mid-forties. This is largely due to declining chromosomal integrity of the eggs with advancing age…”a wear and tear effect” on eggs that are in the ovaries from birth.
2.Embryo Quality/”competency (capable of propagating a viable pregnancy)”. As stated, the woman’s age plays a big role in determining egg/embryo quality/”competency”. This having been said, aside from age the protocol used for controlled ovarian stimulation (COS) is the next most important factor. It is especially important when it comes to older women, and women with diminished ovarian reserve (DOR) where it becomes essential to be aggressive, and to customize and individualize the ovarian stimulation protocol.
We used to believe that the uterine environment is more beneficial to embryo development than is the incubator/petri dish and that accordingly, the earlier on in development that embryos are transferred to the uterus, the better. To achieve this goal, we used to select embryos for transfer based upon their day two or microscopic appearance (“grade”). But we have since learned that the further an embryo has advanced in its development, the more likely it is to be “competent” and that embryos failing to reach the expanded blastocyst stage within 5-6 days of being fertilized are almost invariably “incompetent” and are unworthy of being transferred. Moreover, the introduction into clinical practice about a decade ago, (by Levent Keskintepe PhD and myself) of Preimplantation Genetic Sampling (PGS), which assesses for the presence of all the embryos chromosomes (complete chromosomal karyotyping), provides another tool by which to select the most “competent” embryos for transfer. This methodology has selective benefit when it comes to older women, women with DOR, cases of unexplained repeated IVF failure and women who experience recurrent pregnancy loss (RPL).
3.The number of the embryos transferred: Most patients believe that the more embryos transferred the greater the chance of success. To some extent this might be true, but if the problem lies with the use of a suboptimal COS protocol, transferring more embryos at a time won’t improve the chance of success. Nor will the transfer of a greater number of embryos solve an underlying embryo implantation dysfunction (anatomical molecular or immunologic).Moreover, the transfer of multiple embryos, should they implant, can and all too often does result in triplets or greater (high order multiples) which increases the incidence of maternal pregnancy-induced complications and of premature delivery with its serious risks to the newborn. It is for this reason that I rarely recommend the transfer of more than 2 embryos at a time and am moving in the direction of advising single embryo transfers …especially when it comes to transferring embryos derived through the fertilization of eggs from young women.
4.Implantation Dysfunction (ID): Implantation dysfunction is a very common (often overlooked) cause of “unexplained” IVF failure. This is especially the case in young ovulating women who have normal ovarian reserve and have fertile partners. Failure to identify, typify, and address such issues is, in my opinion, an unfortunate and relatively common cause of repeated IVF failure in such women. Common sense dictates that if ultrasound guided embryo transfer is performed competently and yet repeated IVF attempts fail to propagate a viable pregnancy, implantation dysfunction must be seriously considered. Yet ID is probably the most overlooked factor. The most common causes of implantation dysfunction are:
a.A“ thin uterine lining”
b.A uterus with surface lesions in the cavity (polyps, fibroids, scar tissue)
c.Immunologic implantation dysfunction (IID)
d.Endocrine/molecular endometrial receptivity issues
Certain causes of infertility are repetitive and thus cannot readily be reversed. Examples include advanced age of the woman; severe male infertility; immunologic infertility associated with alloimmune implantation dysfunction (especially if it is a “complete DQ alpha genetic match between partners plus uterine natural killer cell activation (NKa).

Dear Dr. Sher,
I am so glad I found this forum. I am an intended parent and I would like a second opinion.
I ve so far had 2 attempts at IVF transfer. 1st attempt, was only one 5 day embryo with a grading of 4CC. This one failed to implant. Our second attempt was with two 6 day embryos with grading 5BB and 4CC. It resulted in a blighted ovum for both last week at 6th week. We did all the PGS/PGD tests for all the embryos to rule out abnormality before any of the IVFs.
The doctor is now recommending that I change surrogate because he feels there is nothing he can do better on his side and is a bit worried after all these failed attempts.
My surrrogate did have 3 miscarriages before our attempts. She had one before she had her 5 yr old child and 2 more after that. The two after her child were ectopic pregnancies. Both tubes have been removed. Do you think its worth giving another shot? I do feel that the chance of transferring the 2 4cc was a long shot anyhow (but they were the only male embryos). Her uterus lining and everything looked great during each attempt. Its very stressful because I feel the surrogate is really great. But after listening to the doctor, my heart is telling me not to change surrogate yet my head is telling me to. What would be your advise?

I would not go directly to a surrogate. The fact that IVF with PGS-normal embryos could point to a potentially reversible problem that in all probability points to an implantation dysfunction. However, it might be readily reversible obviating the need for a gestational surrogate. In my opinion, the cause of the implantation problem should first be identified and typified before reaching such a drastic and expensive solution.

I am 38 and have one son after a natural FET 2 years ago. I have a low AMH, and since TTC number two, also a borderline lining of 8mm.
I had two chemical pregnancies before my son, but three negative transfers of good embryos this year, one tested and PGS normal. I have done a hysteroscopy and biopsy for NK cells and had some scarring around my C-Section area removed. The blood test for NK cells was low, as was the count in my biopsy.
My doctor has suggested intralipids, but says it isn’t clear what to treat low NK cells with. It may also not mean anything.
What is your opinion of low NK cells? Is it relevant or is it more likely my thinner lining that is the issue? I have one PGS normal blast left and we have decided this will be our last try, so I would like to maximize our chances.

No treatment needed for low NK. However, the blood concentration of NK cells is not relevant. Rather it is the NK cell activity as measured by the K-562 target cell test and/or cytokines that matters.
