My ex husband and I had an alloimmune fertility problem which meant we were unable to have kids together in the years that we were married. I had several miscarriages and then I was unable to get pregnant at all. I have now moved on and have a new partner and we would like to try to have a child together but what are the chances of the alloimmune problem occurring again? And will what happened in the past affect this? Thank you!
Not an alloimmune issue with your new partner, but if you had an alloimmune issue before, you must have had NK cell activation and that will not simply evaporate. So you will need NK cell regulation with intralipid and steroids to conceive together, and probably even IVF because of your residual immunologic implantation dysfunction.

Not sure if this is flare or micro flare protocol: 10 days BCP, Lupron, Gonal F, and then adding menopur. I only took 2 doses of menopur when ultrasound showed lead follicle at 20mm and therefore retrieval was scheduled and I took trigger shot
Retrieval results:
They thought they would get 8 but only were able to retrieve 5.
Only 2 of the 5 were mature. Neither fertilized via ICSI
How would you think about the protocol given those results? I was surprised that the cycle was so short and wonder if a different protocol would yield better results.
I have no uterine issues. I’m 41. I have 2 children ages 2 and 3 which I conceived without issue. I’ve had 4 1st trimester miscarriages in the last 14 months. 2 were trisomy 16 and required d&c and 2 are unknown cause, bleeding naturally at 6 weeks

I am not sure. However, if the Lupron was started with the stimulation then it was a “flare protocol”. In my opinion, the protocol used for ovarian stimulation, against the backdrop of age, and ovarian reserve are the drivers of egg quality and egg quality is the most important factor affecting embryo “competency”.
Older women as well as those who (regardless of age) have diminished ovarian reserve (DOR) tend to produce fewer and less “competent” eggs, the main reason for reduced IVF success in such cases. The compromised outcome is largely due to the fact that such women tend to have increased LH biological activity which often results in excessive LH-induced ovarian testosterone production which in turn can have a deleterious effect on egg/embryo “competency”.
Certain ovarian stimulation regimes either promote excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), augment LH/hCG delivered through additional administration (e.g. high dosage menotropins such as Menopur), or fail to protect against body’s own/self-produced LH (e.g. late antagonist protocols where drugs such as Ganirelix/Cetrotide/Orgalutron that are first administered 6-7 days after ovarian stimulation has commenced).
I try to avoid using such protocols/regimes (especially) in older women and those with DOR, favoring instead the use of a modified, long pituitary down-regulation protocol (the agonist/antagonist conversion protocol-A/ACP) augmented by adding supplementary human growth hormone (HGH). I further recommend Staggered IVF with embryo banking of PGS (next generation gene sequencing/NGS)-normal blastocysts in such cases. This type of approach will in my opinion, optimize the chance of a viable pregnancy per embryo transfer procedure and provide an opportunity to capitalize on whatever residual ovarian reserve and egg quality still exists, allowing the chance to “make hay while the sun still shines”.

Dear Dr. Sher,
First of all, thanks so much for your time and valuable information. I have mild endometriosis and just had an IVF cycle, but the ET had to be postponed and my embryos were frozen. We have 8 high quality embryos that were frozen on day 3. However, after lots of reading, I’ve learned that waiting until day 5 is better for implantation as they become blastocysts. My question is If the lab can culture my embryos till day 5 previous ET (considering they have been frozen on day 3).

I am 13weeks pregnant and I had a vanishing twin syndrome detected at 7wks. A week ago, I experienced a reddish brown bleeding that immediately turned into black. I also experienced cramping. After an ultrasound, my doctor said that it is due to the empty sac from a vanishing twin. The surviving twin is okay and has a consistently good heartbeat. I was advised to rest and was given a medication for the cramping. What is your take on this? Will my surviving baby be okay? Please enlighten me.

I’ve miscarried two euploid embryos and am preparing for a mock FET in advance of another transfer cycle. NK cell activation and cytokines were normal in peripheral blood levels – the only test I haven’t had is an endometrial biopsy for uNK cells or IL-18/TWEAK ratio, to establish levels of intrauterine inflammation. However my nervousness about the biopsy is that levels can bounce up and down each month so I’m unsure as to the usefulness of the test. Would very much welcome your thoughts.
