Can I mix my lupron, gonal-f, HGH and menopur all into 1 single shot?

I am 32 years old and was Diagnosed with low AMH at 31. My amh was a 1.02. They also found a 2cm chocolate cyst on my right ovary. They suspect I have endometrios. I have never had a laproscopy. I did 6 round of clomid with iui and I always responded by making the average or above average amount of follicles.i had one chemical pregnancy from the iui’s. Tubes are open and MRI did not show anything else. I don’t have pain just twinges here and there.
1st IVF- used cetrotide, 225 gonal f and 1 menopur 14 days stims. 12 eggs retrieved 10 mature 8 fertilized natrually and I ended up with 2. One moralua and one blast rated a BB. They said my eggs looked dark though. Transfer was a mess took over an hour and half because my uterus is so tilted back. That was negative. Nothing to freeze
2nd IVF- new doctor long lupron protocol and they increased stims over the 13 days to about 400 gonal f and 1 menopur.
14 eggs 10 mature 9 fertilized, 9 at day 3 and then nothing at day 5. All arrested.
3rd ivf – MDL with Max meds 450 gonal f and 1 vial of menopur. Stimmed for 14 days and was showing 10-12 growing follicles and only retrieved 5 eggs – worst ever. Still waiting on the results.
Do I need donor eggs? Am I wasting my time? Any advice on different protocol?
No! I do not think you need DE. However, there are a few issues to be addressed:
1. The endometriosis is in my opinion creating one definite and one possible problem. The Definite problem relates to the endometrioma. When these are sizable (>1cm) they can activate surrounding ovarian connective tissue causing production of excessive male hormones (androgens) such as testosterone and androstenedione. Excessive ovarian androgens can compromise egg development in the affected ovary (ies) resulting in an increased likelihood of numerical chromosomal abnormalities (aneuploidy) and reduced egg/embryo competency”. In my opinion large ovarian endometriomas need to be removed surgically or rough sclerotherapy before embarking on IVF.
The second is a possible problem. It relates to Endometriosis, regardless of its severity being associated with immunologic implantation dysfunction linked to activation of uterine natural killer cells (NKa) and cytotoxic uterine lymphocytes (CTL) in about 30 of cases. This is diagnosed by testing the woman’s blood for NKa using the K-562 target cell test or by endometrial biopsy for cytokine analysis, and, for CTL by doing a blood immunophenotype. These NKa attack the invading trophoblast cells (developing “root system” of the embryo/early conceptus) as soon as it tries to gain attachment to the uterine wall. In most cases, this results in death of the embryo even before the pregnancy is diagnosed and sometimes, in a chemical pregnancy or even an early miscarriage. . As such, many women with endometriosis, rather than being infertile, in the strict sense of the word, often actually experience repeated undetected “mini-miscarriages”. This requires detailed evaluation and if there is activation of natural killer cells (NKa), this will need to be addressed throu selective immunotherapy with Intralipid and steroids.
2. The second problem is a very important one because it relates to your DOR which, unless addressed by using a very individualized approach to ovarian stimulation, will in my opinion have a persistent adverserial effect on egg and embryo quality.In my opinion, the protocol used for ovarian stimulation, against the backdrop of age, and ovarian reserve are the drivers of egg quality and egg quality is the most important factor affecting embryo “competency”.
Women who (regardless of age) have DOR have a reduced potential for IVF success. Much of this is due to the fact that such women tend to have increased production of LH biological activity which can result in excessive LH-induced ovarian male hormone (predominantly testosterone) production which in turn can have a deleterious effect on egg/embryo “competency”.
While it is presently not possible by any means, to reverse the effect of DOR, certain ovarian stimulation regimes, by promoting excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), can in my opinion, make matters worse. Similarly, the amount/dosage of certain fertility drugs that contain LH/hCG (e.g. Menopur) can have a negative effect on the development of the eggs of older women and those who have DOR and should be limited.I try to avoid using such protocols/regimes (especially) in women with DOR, favoring instead the use of the agonist/antagonist conversion protocol (A/ACP), a modified, long pituitary down-regulation regime, augmented by adding supplementary human growth hormone (HGH). I further recommend that such women be offered access to embryo banking of PGS (next generation gene sequencing/NGS)-selected normal blastocysts, the subsequent selective transfer of which by allowing them to capitalize on whatever residual ovarian reserve and egg quality might still exist and thereby “make hay while the sun still shines” could significantly enhance the opportunity to achieve a viable pregnancy

Thank you for your response. Is Day 14 of the cycle too early to start taking Norethisterone? I read it’s usually day 21, and I was wondering if my nurse made a mistake.
I’ve been told to take it for 14 days starting day 14, and then wait for bleeding to start. Does this sound reasonable?

Doctor in case that the period aftwr overlapping and being since 4 days only with gonapeptyl is only a brown spotting , you suggest waiting till blood finally flow or ir means that that will be the period? (The brown spotting?) I usually have heavy flow that why I ask cause this is new

What I said was that provided the [E2] is low (<70pg/ml) and there are no ovarian cysts, the very 1st sign of bleeding (even if it is light) suggests the beginning of a new cycle.

It depends on the individual case.
