Hi Dr Sher, can antral follicle counts be done at any time of the cycle? Or are there any times where it is not accurate (as I believe they start to die off after a certain amount of time)? Thanks in advance!
It can, but it should ideally be done in the first few days of the cycle.

Hello, there. I just completed a round of IVF that was a failure. My AMH is 4.1, my AFC is 30, my BMI is 18, and my HSG is clear. My periods are regular and I ovulate monthly. When beginning IVF, it appeared as if we would be successful since MFI was our only diagnosis. My husband had testicular cancer this year an a low count in all areas (severe). We retrieved 21 eggs, only 8 mature. Of the 8 that were ICSI’d, 6 fertilized. All made it to day 3 (1 10 cell, 4 8 cell, and 1 5 cell). They decided to do a 5 day transfer. We got the call on the day of the transfer that all embryos arrested and there would be no transfer. We are devastated. They claimed that the embryologist saw some darkening and granular areas on the eggs and that they presumed egg quality was the issue.
We were on the antagonist protocol, no BCP, follistim (150 u), menopur a few days in (75 u), ganirelix (day 4). They triggered with 1,000 units of HcG (Pregnyl) and 80 units of Lupron. Do you have any insight why an otherwise healthy 27 year old, given this information, would have such a devastating end of a first IVF attempt? Is this something that will continue to happen?
It is possible that the protocol used for stimulation might be the problem, especially since you had a Lupron “trigger”.. (see below). I would love to discuss this in detail with you. It is also possible that MF could have played a significant role given your husband’s history.

Hi Dr. Sher. Does grading even matter if embryos are pgs normal? I have two day 5 blasts that are graded 3BB but PGS normal. My RE says they aren’t the best grade but do turn into pregnancies. What are the chances both can implant and become twins? These embryos were frozen at my age of 34. I won’t use them until age 38 or so when we’re ready to add to the family. I have twins now from day 5 blasts. Thanks.
Morphologicgrading should not matter, provided the testing was done accurately. And yes, both could take.

I’m 37 years old, I have 2 boys 13 and 15. At age 26 I had my tubes cut and now I underwent my first IVF in hopes of having a baby girl. I had 9 eggs extracted on 2/21/17 and 3-day embryos transferred. I underwent a blood test 7 days after transfer and I got the results today with a <2 Hcg therefore, negative. Did I test too early? I also had intercourse on day 7 after ET. I feel guilty. Did I mess everything up? Thank you for your time.
It is about 3 days too early. Repeat the test!

What do you think of the new ERA biopsy test for uterine receptivity? Do you recommend it and in what cases?
I am in no way convinced of a benefit and I do not use it presently.
