  1. Dear Dr Sher,
    I am two days after FET and my clinic still wants me to take 4×1 Sildenafil vaginally – is it safe for the two embryos already transfered?

    I never had very thin lining -between 8 and 9 . But I have a history of many unsuccessful FET attempts with good embryos and no NK activation. But my lining usually grew steadily to around 9mm . But this time…. I am worried that this viagra thing made my lining slow down because on cd 9 it was 7.5 and normally by cd 13 it would grow by more than current 0.5mm
    Below is how it grew with use of Sildenafil :

    (Here it was decided to start Sildenafil on cd11 and it seems like it slowed down the growth of lining…)

    Endo lining was only 8.1.

    I would love to here your opinion:
    1. During first week I was taking vaginally regular pills that didn't disolve very well and caused excessive dryness and swelling). Only the fourth day now I am taking specially formulated Sildenafil for vaginal use (those disolve immediately! )- could those first pills negatively affect my lining?

    2. Is it safe to take Sildenafil vaginally after the transfer – and if so, for how long?

    3. And last question : for few days after transfer do you recommend bed rest or rather moving around to get the blood flaw?

    • The viagra needs to be specially compounded for vaginal administration and must be used for at least 2-3 days prior to the trigger shot or commencement of progesterone therapy.. It has no benefit if used after this.

  2. Dr Sher,

    On my lining check today my endometrium is only 7.4mm. My transfer is in 6 days.

    1. Should I cancel the transfer or go ahead?

    2. Is it too late to increase the estrogen dose?

    3. Will starting the progesterone shots tonight help my lining?

    • I cannot dictate what you should do. However, I personally do not do an ET unless the lining is >8mm on the day of the ‘trigger shot” or on the day progesterone therapy is initiated when an FET is done.

  3. Dear Dr. Sher,
    you already helped me with my case and advised me to combine IL with steroids in an autoimmune implantation dysfunction with increased NK cell activity. You told me to start the treatment of both about 6 days prior to ovulation. As I become pregnant spontaneously, I’m not sure how to move on with the dexamethasone dose. I started with 0,75mg. Am I supposed to rise the dose with ovulation or positive HCG blood test?

    • If I were treating you, I would advise that you continue the 0.75mg dexamethasone daily to the 8th week and then tail it off slowly . stopping by the 10th week. Discuss with your treating doctor first.

  4. Hi. I’m 43 yo and have had IVF 4 times and multiple IUI’S and I get pregnant and lose it very early on. Would love to hear what protocol you use on your pts. I go to CNY in Latham and just would like to hear your opinion. My hubby is 49 with retrograde ejaculation.

    I am 8 days past a 6 blastocyst donor egg Fet. I’ve had 2 failed transfers previous to this. I purchased 4 first response early detection home pregnancy tests and tested on wednesday, yesterday and today, all have come back positive. 2 lines one line slightly lighter. But 2 clearly defined lines with a darker second line today. How accurate are these hpt tests. My hcg beta blood test is scheduled for Wednesday April 5th. How probable is it that I will be receiving good news? I have not required a trigger shot as I did a donor egg ivf cycle. I am on extrace, progesterone in oil shots, prednisone, and fragmin and baby aspirin.
