Hi Dr Sher,
We are considering NK cell testing. Following our research is seems most clinics in London test the following:
1. NK cells activation marker CD-69
2. NK cells cytotoxicity assay
3. Uterine NK cell count
What are your thoughts on the above? Do you have any further recommendations? We are already booked in for: • Hysteroscopic evaluation of the uterine cavity • Full hormonal evaluation (estrogen, progesterone, adrenal steroid hormones, thyroid hormones, FSH/LH, etc.)
Thank you so much!
In my opinion, the best test that reliably assesses NK cell activity is the K-562 target cell test. Uterine cytokines is the only other one I would take into consideration.

Hello Sir,
First Beta HCG tested after 13 days post Blastocyst Transfer (2 Blastocysts were transferred then). The Beta HCG result is 2087.76 mIU/mL. Could you tell me what to infer from this result?
Also, I am on 8% Crinone Gel (BD), 50 mg/ml Gestone Injection (OD) and 10 mg Duphaston Tablets (BD). Is there a further need to monitor my blood progesterone levels and increase the daily dose of progesterone?
Thanks & Regards,
–Mrs.Saana Ghosh
That is a high level…could be a multiple pregnancy. Repeat it in 2 dys.
Good luck!

Hi doctor
We are married for 8 years. I concieved once in 2012 and again 2013. Both ended up as blighted ovum. We have done all blood tests, karyotyping and hysteroscopy and everything came as normal. For the past one year we have done 3 IUI and they weren’t successful. Finally we went for ivf. Dr transferred 3 embryos which are day 3. After two weeks hcg came as 45 then 86. Post a week, it was 280 and 370 after 2 days. But unfortunately bleeding started and hcg became 160. Doctor said this is case of pregnancy out of unknown origin. What else do you suggest us. What course of action we should take to have a healthy baby. Thanks a lot in advance
Dr. Sher,
We had a pgs normal donor embryo FET that failed to implant (failed pregnancy test). FSH prior to the FET was 27.5. Does FSH play a role in implantation? After going through an ERA cycle (waiting for results) I am now preparing for my next FET and as of today have a FSH of 19.9. Just wondering how/if FSH plays into FET. Also, have you had any success due to ERA? Scratch?
Thank you,
Paulina
The FSH plays no role in FET. Respectfully, I do not believe thatv ERA has value clinically.

Hi dr. sher, i am awaiting to start a frozen cycle and need to go in on day 2/3 to then start BCP and hopefully transfer in 6 weeks. My question is last cycle I only spotted for 4 days , never a full flow and now this cycle seems to be only spotting as well. I have never had this happen before. My question is will the effect my being able to proceed with my frozen cycle? And why would my cycle change so drastically all of a sudden? I will go in tomorrow to see my doctor but wanted to get your opinion
Reduced bleeding with cyclical menses is not necessarily a measure of hormonal dysfunction. It should not affect your ability to proceed, unless you are found to have an ovarian cyst…in my opinion.
