Dear Dr Sher , for Immunologic implantation dysfunction , what is the Intralipids and the steroids protocol please? Thank you for sharing
100cc, 20% IL diluted in 500ccof normal saline and infused 10-14 days prior to ET…over 3-4 hours. Repeated with the +ve beta hCG test and in cases of alloimmune implantation dysfunction (partial DQ alpha match) every 2-4 weeks thereafter until the 24th week of pregnancy. Dexamethasone 0.75mg orally daily from the beginingof ovarian stimulation until the 8th week of pregnancy and tailed off in the ensuing 2 weeks…or earlier if pregnancy does not occur or fails.
Good luck!

Hi Dr. Sher,
I’m 43 and turning 44 end of November. I’m taking CO-Q10, DHEA, and some parts of the month black cohosh (first 10 days). I have a high BMI which I will bring DOWN to 43 by the summer. I have trouble with my weight, but I’ve just lost about 10 lbs and so I see hitting my summer goal.
I’ve completed 4 failed IUIs, and have two more prior moving to IVF. Two of those completed were clomid 100 cycles with Crinone (progesterone).
I am hopeful for IVF considering the steps I’m taking. I would love to use my my own eggs, but I’ve asked my sister if she would donate as a backup plan (she’s 38), if she agrees, it would be great. She has not yet agreed and is thinking about the moral/personal reasons.
My questions are:
1. given my stats, how much does weight loss impact the developing eggs? How soon will weight loss take effect (do I need to finish getting to the lower BMI before effects take place?)
And 2. other than weight loss and Co-Q10 + supplements, what else should I be doing to improve egg quality/environment? I like yoga, swimming, cycling.
And 3. Insurance will only cover IVF till age 44. Any recommendation on # cycles of IVF with my own eggs should I use before going to donor egg?
My questions are:
1. given my stats, how much does weight loss impact the developing eggs? How soon will weight loss take effect (do I need to finish getting to the lower BMI before effects take place?)
A: It is a good goal to try and get your BMI under 25 but age and its adverse influence on on egg/embryo competency and unfortunately that is inevitable.
And 2. other than weight loss and Co-Q10 + supplements, what else should I be doing to improve egg quality/environment? I like yoga, swimming, cycling.
A: Unfortunately age is the factor. Exercise and life-style changes , although commendable will not likely improve egg quality.
And 3. Insurance will only cover IVF till age 44. Any recommendation on # cycles of IVF with my own eggs should I use before going to donor egg?
If you decide to use your own eggs, my recommendation would be a modified, long pituitary down-regulation protocol. I would use an agonist/antagonist conversion protocol with human growth hormone (HGH) augmentation and would recommend Staggered IVF with embryo banking of PGS (next generation gene sequencing)-normal blastocysts, to make hay while the sun still shines.

Good morning Dr. Sher, can muti fibroid cause preterm labor at 20 weeks? 4 is outside the uterus, 1 is in the wall of the uterus and they are all between 5 – 6 cm. Thanks
Yes it can do!

Hello! I’m seeking for advice. I’m 31 and my husband is 32. We’re healthy and live a pretty healthy lifestyle. We’ve been trying to have a baby for 2,5 years. We’ve done 2 IUI and 3 IVFs. I’ve never been pregnant. I have only one ovary since the other one was removed as a baby due to ovarian torsion. Also they had to remove a cyst from my other ovary as well. My AMH level is only 0.3 and my eggs seem to be bad quality. Our embryos are slow and stop dividing. On the first IVF we had 8 mature eggs, 5 fertilized, 4 made it to day 3 but they were slow and never made it to morula or blastocyst stage. IVF 2 we didn’t get to egg retrieval. IVF 3 we had 8 mature eggs, 6 fertilized, 5 made it to day 3. This time on day 3 there were two 8-cell embryos, others were slow again. On day five the other 8-cell embryo had slowed down. The other one was on morula stage, so a bit slow too. We transfered the morula embryo, no pregnancy. So our embryos never make it to blastocyst. What should we do? Is there any chance with my own eggs? Any advice? I’m not sure if I want donor eggs or embryos.
It is in my opinion its all about using the ideal protocol for ovarian stimulation. I would suggest a modified, robust, long pituitary down-regulation protocol. I would use an agonist/antagonist conversion protocol with human growth hormone (HGH) augmentation and would recommend Staggered IVF with embryo banking of PGS (next generation gene sequencing)-normal blastocysts, to make hay while the sun still shines.
