Dear Dr Sher,
I have a trigger shot question. In the past I used 10000 pregnyl as a trigger having read your blog. Even at this dosage I had a high number of immature eggs. My n8-9 Dr suggests I used 250 ovitrelle plus 0.2 Mg decapeptyl as he thinks the Decapetyl may help trigger better meiosis. What’s your opinion of this combo ovitrelle and decapeptyl? I am doing mild IVF and am expecting about 8-9 eggs as per last time (3-4 were mature).
In my opinion, the trigger shot of 10,000U hCG is adequate. In most such cases, it is the method used for ovarian stimulation (to set up the “trigger”) that needs to be reviewed/revised (see below).

I have had 1 successful birth with donor eggs transferring 2 fresh day 5 embryos resulting in a singleton. She’s 3 now. We started trying for #2 1 year ago and did 3 more frozen transfers from embryos on that first donor cycle. 2 chemical pregnancies and 1 negative. We switched donors and did pgd testing of the embryos. We have done 2 frozen transfers both have resulted in negative pregnancy tests. Lining as well as estradiol and progesterone levels have all even normal each cycle. Saline infusion sonos have all been clear and I have had 2 hysteroscopy’ so to make sure nothing in there causing problems. The last hysteroscopy was before the last 3 transfers. We have checked for clotting disorder and nothing is off there. Not sure what the reason for failure is with pgd tested embryos. Can you have an immunology problem if you have had previously successful pregnancy? And is there anything to suggest transferring 2 is the trick to beating an immunology issue or was my first successful pregnancy the exception? We have 3 pgd tested embryos left and don’t want to waste them.

Yes you absolutely can have a 2ndary immunologic problem, especially relating to alloimmune implantation dysfunction (please read below).
Whenever a patient fails to achieve a viable pregnancy following embryo transfer (ET), the first question asked is why! Was it simply due to, bad luck?, How likely is the failure to recur in future attempts and what can be done differently, to avoid it happening next time?.
It is an indisputable fact that any IVF procedure is at least as likely to fail as it is to succeed. Thus when it comes to outcome, luck is an undeniable factor. Notwithstanding, it is incumbent upon the treating physician to carefully consider and address the causes of IVF failure before proceeding to another attempt:
1.Age: The chance of a woman under 35Y of age having a baby per embryo transfer is about 35-40%. From there it declines progressively to under 5% by the time she reaches her mid-forties. This is largely due to declining chromosomal integrity of the eggs with advancing age…”a wear and tear effect” on eggs that are in the ovaries from birth.
2.Embryo Quality/”competency (capable of propagating a viable pregnancy)”. As stated, the woman’s age plays a big role in determining egg/embryo quality/”competency”. This having been said, aside from age the protocol used for controlled ovarian stimulation (COS) is the next most important factor. It is especially important when it comes to older women, and women with diminished ovarian reserve (DOR) where it becomes essential to be aggressive, and to customize and individualize the ovarian stimulation protocol.
We used to believe that the uterine environment is more beneficial to embryo development than is the incubator/petri dish and that accordingly, the earlier on in development that embryos are transferred to the uterus, the better. To achieve this goal, we used to select embryos for transfer based upon their day two or microscopic appearance (“grade”). But we have since learned that the further an embryo has advanced in its development, the more likely it is to be “competent” and that embryos failing to reach the expanded blastocyst stage within 5-6 days of being fertilized are almost invariably “incompetent” and are unworthy of being transferred. Moreover, the introduction into clinical practice about a decade ago, (by Levent Keskintepe PhD and myself) of Preimplantation Genetic Sampling (PGS), which assesses for the presence of all the embryos chromosomes (complete chromosomal karyotyping), provides another tool by which to select the most “competent” embryos for transfer. This methodology has selective benefit when it comes to older women, women with DOR, cases of unexplained repeated IVF failure and women who experience recurrent pregnancy loss (RPL).
3.The number of the embryos transferred: Most patients believe that the more embryos transferred the greater the chance of success. To some extent this might be true, but if the problem lies with the use of a suboptimal COS protocol, transferring more embryos at a time won’t improve the chance of success. Nor will the transfer of a greater number of embryos solve an underlying embryo implantation dysfunction (anatomical molecular or immunologic).Moreover, the transfer of multiple embryos, should they implant, can and all too often does result in triplets or greater (high order multiples) which increases the incidence of maternal pregnancy-induced complications and of premature delivery with its serious risks to the newborn. It is for this reason that I rarely recommend the transfer of more than 2 embryos at a time and am moving in the direction of advising single embryo transfers …especially when it comes to transferring embryos derived through the fertilization of eggs from young women.
4.Implantation Dysfunction (ID): Implantation dysfunction is a very common (often overlooked) cause of “unexplained” IVF failure. This is especially the case in young ovulating women who have normal ovarian reserve and have fertile partners. Failure to identify, typify, and address such issues is, in my opinion, an unfortunate and relatively common cause of repeated IVF failure in such women. Common sense dictates that if ultrasound guided embryo transfer is performed competently and yet repeated IVF attempts fail to propagate a viable pregnancy, implantation dysfunction must be seriously considered. Yet ID is probably the most overlooked factor. The most common causes of implantation dysfunction are:
a.A“ thin uterine lining”
b.A uterus with surface lesions in the cavity (polyps, fibroids, scar tissue)
c.Immunologic implantation dysfunction (IID)
d.Endocrine/molecular endometrial receptivity issues
Certain causes of infertility are repetitive and thus cannot readily be reversed. Examples include advanced age of the woman; severe male infertility; immunologic infertility associated with alloimmune implantation dysfunction (especially if it is a “complete DQ alpha genetic match between partners plus uterine natural killer cell activation (NKa).

My husband and I were ttc unsuccessfully for 2 years before doing 4 cycles of IUI and ultimately 1 cycle of IVF which was successful with elective single embryo transfer and 4 great quality blastocysts to freeze (4 years ago). The pregnancy was healthy and no complications with delivery. We decided to try for another baby this year and did a FET with our 5AA quality blastocyst which resulted in a chemical pregnancy. During the week that I was pregnant I developed some sort of histamine response- random hives and itching all over my body and hypersensitivity to the progesterone injections (hot and very swollen welts) and the estrogen patches. Around the time that the pregnancy failed all histamine symptoms subsided. No more reactions to the injections, no more welts, etc. We tried again with our next embryo which was 5AB quality. The day following transfer I developed flu like symptoms- achy joints, extremely tired feverish feeling. This lasted the entire day following the FET. We have just discovered that this cycle has failed. I am concerned that I am having some sort of immunological response to embryo implantation although my symptoms were different both cycles. I have 2 frozen embryos left and I am concerned that I should have testing done before trying again. What kind of tests do you recommend having done and where (if any)? Any other advice?

Here is my history: I am 29 (almost 30), husband is 33 (almost 34). My AMH last year was 1, day 3 FSH 9. I had a hydrosalpinx to left fallopian tube and had left tube removed on 8/6/15, no complications but did find a very minimal small amount of endometriosis. All other labs were normal. Husband sperm counts were normal. We have done 4 IUIs and 3 IVFs.
My question is regarding your thoughts/opinion on protocol for my next IVF cycle. We are trying to decide between doing another IVF with the same protocol as my last IVF or doing a cycle similar to my IUI#4. Please see below statistics and let me know your thoughts:
IUI#4: Follistim 150 iu on cycle days 3-7
Baseline CD2: AFC was 15
CD8 check: follicles measuring 10mm, 11 (x2), 12, 13 (x4), 15, 16, and 20mm // Estradiol 1073
**I was triggered that night with likely 3 mature follicles, because my RE did not feel comfortable pushing me farther and having more mature follicles. I am wondering if we had added in ganerelix if we could have possibly had a really great cycle for a retrieval instead of doing an IUI…
IVF #3: Lupron Flare protocol (started Lupron 20iu on day 1, decreased to 5iu on day 5 and continued until trigger; started low dose Hcg 20iu on day 2 and continued until trigger; started follistim 300iu BID on day 2 and continued until trigger.)
Baseline CD2: AFC 22
Cycle Day 9: follicles measuring 10, 11 (x2), and 12 (plus some small unmeasurable ones). //e2 660
Cycle Day 11: follicles 10, 11, 12, 13, 16, 17mm // e2 1239
Cycle Day 13: follicles 13 (x2), 14, 15, 17, 20, 22 //e2 2923
Cycle Day 14: follicles 11, 12 (x3), 15 (x2), 16, 19, 22, 23 // e2 3154 — triggered tonight
Retrieved: 18, 8 mature, 6 fertilized with ICSI.
Day 3: all 6 embryos still growing — 11 cell (grade B), 8 cell (grade A) x2, 6 cell (grade B), and 4 cell (grade C) x2.
Day 5: only one blastocyte perfect grade frozen. No other embryos made it to freeze.
When reviewing the two above protocols and my response, I wonder if it would be worth trying an FSH only stimulation to see if can attain better quality eggs although we likely will not retrieve the same amount. I was hoping you may be able to provide some insight and your opinion of my responses to each protocol and also which you think would be best to move forward with for my next IVF retrieval. Thank you in advance for you time and dedication.
Your AMH of 1.0ng/ml is on the low side. This in conjunction with the high stimulation required suggests to me that suggesting that your ovarian reserve may be declining and wome who have diminished ovarian reserve (DOR) tend to have increased LH biological activity which can result in excessive LH-induced ovarian testosterone production (often not detectable in peripheral blood ) which in turn can have a deleterious effect on egg/embryo “competency”.
In my opinion, certain ovarian stimulation regimes either promote excessive LH production (e.g. short agonist/Lupron- “flare” protocols, clomiphene and Letrozole), augment LH/hCG delivered through additional administration (e.g. high dosage menotropins such as Menopur), or fail to protect against body’s own/self-produced LH (e.g. late antagonist protocols where drugs such as Ganirelix/Cetrotide/Orgalutron that are first administered 6-7 days after ovarian stimulation has commenced).
I try to avoid using such protocols/regimes (especially) in older women and those with DOR, favoring instead the use of a modified, long pituitary down-regulation protocol (the agonist/antagonist conversion protocol-A/ACP) augmented by adding supplementary human growth hormone (HGH).
Also, with a history of unilateral hydrosalpinx it is highly likely that BOTH your tubes were damaged (even if one was more severely affected than the other) and I would not recommend IUI in such cases, preferring to go directly to IVF.

Heyy Dr. Sher I have several questions plz:
1) My husband has secondary hypogonadism (since he was 10 and now He is 24 years old) his abnormal sperms rate is 5%, so is he more likely to have a high sperm DNA fragmentation???
2) And if we did IVF/ICSIS and the transfered embryo was fertilized with an abnormal sperm, the chance to have face miscarriage or an unhealthy/abnormal child will be likely to happen??

A: Not necessarily, but depending on the degree and severity of the sperm dysfunction the success with ICSI might be lower than optimal.
