Hi Dr Sher, is supplanting agonist with 1/2 dose antagonist a few days after a menstrual period arrives fatal? Or is it best for the switch to occur on day of menstrual period?
No it is not fatal but the sooner it is started after the onset of menses, the better.

My RE didn’t give me any estrogen support after a fresh transfer. Just progesterone. My RE says estrogen is not important and I know a lot of women who only get progesterone, never estrogen. What effect does estrogen have at all during pregnancy?
There is a difference of opinion when it comes to fresh ET’s and there is no convincing data, one way or the other. However, in my long experience, I favor using both E2 and progesterone.

Ultrasound at 5 weeks 5 days show two gestational sacs with no yolk sacs. Is it just too early? Thank you for answering.

My beta hcg level is 5.14umi/ml am I pregnant?
That is a low number and cause for concern. Repeat it in 2 day intervals to see if it doubles each time.

I’m currently cycling abroad and the progesterone suppository Rx here is huge (200mg x3/day) compared to the cycle I did back home (100mg x2/day). Drs here aren’t very communicative- is it safe to be on this much progesterone until I’m 12 weeks? I’m currently in the 2ww and will consult with a Dr back home too but trust your opinion so much. Is it safe to gradually taper off progesterone after a positive beta to say 200mg/day after having been on a 600mg/day dose, or do I have to keep going on this high dose until my first trimester ends? The side effects on this dose are so uncomfortable
It is probably safe but may not be necessary. I stop progesterone abruptly without tapering it off…at 10 weeks.
